The MTHFR topic deserves careful wording. Common MTHFR variants can affect folate metabolism, but they do not automatically mean someone needs very high-dose methylfolate. Many people with MTHFR variants meet folate needs through diet, standard prenatal dosing when appropriate, or ordinary supplement amounts. High-dose methylfolate can still push folate markers high, and it should be treated like an active supplement, not as a harmless workaround.
Less often, a high folate result reflects the timing of the blood draw, inconsistent supplement use, lab reporting limits, or a mismatch between the test ordered and the question being asked. For example, serum folate may be high because the person took a vitamin recently, while the real clinical question is whether B12 status is adequate.
High folate is usually less concerning when it is clearly explained by intake and other labs are reassuring. It becomes more important when the person has anemia symptoms, nerve symptoms, cognitive changes, risk factors for B12 deficiency, or very high supplement doses.
Folic Acid, Supplements, and Safe Upper Limits
Folate from food and folic acid from supplements are related, but they are not handled the same in safety guidance. Natural food folates are found in foods such as spinach, lentils, black-eyed peas, asparagus, Brussels sprouts, avocado, oranges, and liver. Folic acid is the synthetic form used in many supplements and fortified foods. Some supplements use 5-MTHF, also called methylfolate or L-methylfolate.
Adults generally need 400 mcg dietary folate equivalents, or DFE, per day. During pregnancy, the recommended intake rises to 600 mcg DFE per day, and during lactation it is 500 mcg DFE per day. People who may become pregnant are often advised to take folic acid because the neural tube forms very early, often before someone knows they are pregnant.
The adult tolerable upper intake level is 1,000 mcg per day for folic acid from supplements and fortified foods. This upper limit is not the same as the recommended intake. It is a safety boundary for regular unsupervised intake. It also does not apply to natural food folate from ordinary foods.

B12 Masking and Why It Matters
High folate matters most when vitamin B12 deficiency is possible. Folate and B12 work in connected pathways. Both are needed for healthy red blood cell production and DNA synthesis. When either one is low, red blood cells can become large, a pattern called macrocytosis, and a person can develop megaloblastic anemia.
Large amounts of folic acid can improve the anemia caused by B12 deficiency without fixing the B12 problem in the nervous system. This is the classic “B12 masking” concern. The blood count may look better while numbness, tingling, balance trouble, memory changes, or other neurological symptoms continue. Some nerve injury from untreated B12 deficiency can become lasting, so the masking concern is not just a lab technicality.
B12 deficiency can also occur without anemia. A normal hemoglobin or normal MCV does not fully rule it out, especially in older adults, people who follow vegan diets without reliable B12 intake, people with pernicious anemia, people after stomach or intestinal surgery, and people who use metformin or acid-suppressing medicines long term.
A high folate result should prompt a B12 check when any of these apply:
- Numbness, tingling, burning feet, balance problems, or reduced vibration sense
- Memory changes, confusion, depression, or unusual irritability
- Glossitis, mouth soreness, fatigue, palpitations, shortness of breath, or pale skin
- Vegan or mostly plant-based diet without consistent B12 supplementation
- Older age, especially with low stomach acid or atrophic gastritis
- Pernicious anemia or autoimmune gastritis
- Celiac disease, Crohn’s disease, gastric bypass, ileal surgery, or other malabsorption risk
- Long-term metformin use

The most helpful follow-up is often not B12 alone. Serum B12 can be low, borderline, normal, or occasionally misleading. Methylmalonic acid, often shortened to MMA, rises when B12-dependent metabolism is impaired. Homocysteine may rise with low folate, low B12, low vitamin B6, kidney dysfunction, and other factors. High folate does not cause B12 deficiency. The concern is that high folate can hide or blur one of the clues. If B12 deficiency is present, the treatment is B12 replacement and evaluation of the cause, not simply lowering folate.
How to Follow Up on a High Result
Follow-up starts with the exact result. Look at the test name, number, unit, reference range, and whether the lab measured serum folate or RBC folate. A result reported as “>20 ng/mL” is different from a precise value, and a serum folate result is different from an RBC folate result.
Next, list every possible folate source. Include prescription products, multivitamins, prenatal vitamins, B-complex, methylfolate, folinic acid, energy drinks, fortified cereals, protein powders, meal replacements, fertility products, and “methylation” formulas. - Check whether B12 was tested at the same time.
- Review CBC markers, especially hemoglobin, hematocrit, MCV, RDW, white blood cells, and platelets.
- Ask whether MMA and homocysteine are needed, especially if B12 is low, borderline, or symptoms are present.
- Decide with a clinician whether to repeat the test after holding nonessential supplements.
A repeat test can be useful when the result is unexpected. Many clinicians prefer repeating serum folate after the person avoids nonessential folate-containing supplements for a short period, but this should not include stopping prescribed folate, prenatal vitamins, or medication-related folate without medical advice.
CBC patterns can add context. Folate deficiency and B12 deficiency can both cause macrocytosis, which means high MCV. A high folate result should not be treated as proof that fatigue, brain fog, neuropathy, or anemia has nothing to do with nutrients. It only says folate in the tested compartment is high. Iron status, B12 status, thyroid markers, kidney function, liver markers, inflammation, and medication effects may still be relevant.
When High Folate Needs Medical Attention
High folate needs medical attention when the result is paired with symptoms, abnormal blood counts, B12 risk factors, pregnancy questions, or high-dose supplement use. The folate number alone is rarely the main issue. The surrounding pattern is what changes the level of concern.
Contact a clinician promptly if high folate appears with nerve-related symptoms, especially numbness, tingling, burning, weakness, balance problems, falls, or trouble walking. These symptoms can occur with B12 deficiency and should not be explained away by a normal or improved blood count.
Medical review is also important when high folate appears with:
- Low or borderline vitamin B12
- High MMA
- High homocysteine

Pregnancy deserves separate care. Folic acid before and during early pregnancy has a well-established protective role. A high folate result in someone taking a prenatal vitamin may not mean the prenatal is wrong. However, taking multiple prenatal products, a prenatal plus a B-complex, or high-dose methylfolate on top of fortified foods can produce unnecessarily high intake. This is worth discussing with an obstetric clinician, especially if the dose exceeds standard recommendations.
Medication context also matters. Methotrexate is a folate antagonist, and folic acid or folinic acid may be used differently depending on whether methotrexate is being used for cancer, rheumatoid arthritis, psoriasis, or another condition. High folate without symptoms and with clear supplement exposure is often handled by dose review rather than urgent testing. High folate with neurological symptoms, abnormal CBC results, or uncertain B12 status deserves more careful evaluation.
How to Lower an Unnecessarily High Folate Level
Lowering high folate usually means reducing unnecessary supplemental folate, not avoiding healthy foods. Most people should not cut out leafy greens, beans, lentils, asparagus, citrus, avocado, or other nutrient-dense foods just because serum folate is high. Food folate is part of a healthy diet and is not the usual source of excessive synthetic folic acid exposure.
The first step is to identify duplicate supplements. Many people take several products that each contain folate. A multivitamin may contain 400 mcg DFE. A B-complex may add another 667 to 1,700 mcg DFE. A methylfolate capsule may add 1,000 mcg or more. A fortified cereal may add folic acid at breakfast. The total can climb quickly.
Common ways to reduce unnecessary intake include:
- Stop duplicate B-complex or “methylation” products if they were not prescribed.
- Choose one multivitamin or prenatal rather than stacking several formulas.
- Avoid high-dose methylfolate unless there is a clear reason and follow-up plan.
- Check energy drinks and fortified shakes for added folic acid.
- Use standard-dose folate for general nutrition unless a clinician recommends more.
- Keep prescribed folate plans in place until the prescribing clinician advises otherwise.
If a clinician agrees that a supplement is unnecessary, serum folate may fall fairly quickly after stopping or lowering the dose because serum folate reflects recent intake. RBC folate changes more slowly because it reflects folate inside red blood cells.
People sometimes worry that lowering folate will create deficiency. That is unlikely if the diet is varied and the person continues an appropriate standard-dose supplement when needed. Deficiency is more likely in people with heavy alcohol use, malabsorption, very limited diets, certain medications, increased needs, or poor overall intake. If deficiency risk exists, the plan should balance avoiding excess with maintaining adequate folate.
It is also important not to treat high folate as a standalone supplement problem if the symptoms point elsewhere. Fatigue, shortness of breath, palpitations, dizziness, neuropathy, mood changes, and cognitive symptoms can have many causes. A high folate result may be one clue, but it should not distract from checking B12, iron markers, thyroid function, kidney function, inflammation, and the CBC when clinically appropriate. A sensible target is not “as low as possible.” It is adequate folate without unnecessary megadosing, especially when B12 status is uncertain.
