
Triglycerides are fats that travel in the blood and store energy for later use. Most people hear about triglycerides when the result is high, but a low triglyceride blood test can also raise questions. In many healthy adults, low triglycerides are not dangerous, especially when the rest of the lipid panel looks good and there are no symptoms. A low result can simply reflect recent fasting, a lower-carbohydrate diet, regular exercise, weight loss, or medication that lowers blood lipids.
Very low triglycerides deserve more attention when they are persistent, unexpected, or paired with symptoms such as unintentional weight loss, diarrhea, poor appetite, tremor, heat intolerance, fatigue, or signs of nutrient deficiency. The result should be interpreted with total cholesterol, LDL cholesterol, HDL cholesterol, VLDL cholesterol, non-HDL cholesterol, ApoB, glucose markers, thyroid testing, liver tests, medications, diet pattern, and overall health.
- Low triglycerides are often benign when levels are mildly low, the person feels well, and other lipid markers are normal.
- A common “low” cutoff is below 50 mg/dL, although many labs do not flag low triglycerides because medical concern is usually focused on high levels.
- Very low triglycerides below about 30 mg/dL are more likely to deserve repeat testing and a search for causes, especially if LDL cholesterol, total cholesterol, or VLDL cholesterol are also very low.
- Common causes include fasting, low-carbohydrate or very-low-fat diets, weight loss, exercise, lipid-lowering medication, hyperthyroidism, malnutrition, malabsorption, chronic illness, and rare genetic lipid disorders.

What Low Triglycerides Mean
Low triglycerides mean the amount of triglyceride fat circulating in the blood is lower than expected. Triglycerides are made of three fatty acids attached to glycerol. After a meal, the intestine packages dietary fat into chylomicrons. Between meals, the liver sends triglycerides into the bloodstream inside very-low-density lipoprotein, or VLDL, particles. These particles deliver fatty acids to muscle, fat tissue, and other organs.
A low value usually means there is less triglyceride-rich lipoprotein in the blood at the time of testing. That can happen for normal reasons. A person who fasted longer than usual, ate fewer calories, lost weight, trained heavily, or ate a low-carbohydrate diet may have a lower result. Some people naturally run low because of genetics and still remain healthy.
Low triglycerides are different from low cholesterol. Triglycerides measure circulating fat used mainly for energy transport. Cholesterol measures sterol molecules carried inside lipoproteins and used for cell membranes, bile acids, and hormone production. The two often move together because they travel in related particles, but they are not the same marker. A person may have low triglycerides with normal LDL cholesterol and HDL cholesterol, or low triglycerides with low total cholesterol and low LDL cholesterol.
The result is also different from low VLDL cholesterol. VLDL particles carry much of the triglyceride made by the liver, so low triglycerides often produce a low calculated VLDL value. A separate article on low VLDL cholesterol can help when both values are flagged together.
Low triglycerides are usually less concerning than high triglycerides. High triglycerides can signal insulin resistance, diabetes, excess alcohol intake, fatty liver risk, certain medication effects, and pancreatitis risk at very high levels. Low triglycerides, by contrast, often reflect efficient triglyceride clearance or lower triglyceride production. The concern rises when the number is extremely low, persistent, unexplained, or part of a broader pattern of poor nutrition, malabsorption, overactive thyroid, liver disease, or inherited hypolipidemia.
Normal, Optimal, Low, and Very Low Ranges
Triglyceride ranges vary by laboratory, fasting status, age, pregnancy status, and clinical guideline. Many lipid panels focus on the upper end because cardiovascular and pancreatitis risk increase with high triglycerides. As a result, a lab may not mark low triglycerides as abnormal unless they are very low.
For adults, fasting triglycerides below 150 mg/dL are generally considered normal. Many clinicians view levels below 100 mg/dL as metabolically favorable, especially when HDL cholesterol, blood glucose, blood pressure, waist size, and ApoB are also in a healthy pattern. A triglyceride result in the 40–80 mg/dL range can be completely normal in a physically active person or someone eating a diet with fewer refined carbohydrates.
There is no single universal medical cutoff for “too low.” In practice, the following ranges are useful for interpretation:

Fasting status affects triglycerides more than it affects total cholesterol, HDL cholesterol, or many direct LDL measurements. After a meal, triglycerides often rise because the intestine sends fat into the bloodstream. Nonfasting lipid panels are common in routine screening, but a very low nonfasting triglyceride result may simply mean the meal was low in fat, the person had not eaten much, or triglyceride clearance was efficient.
A standard fasting lipid panel is usually drawn after 8–12 hours without calories. Water is allowed. Black coffee rules vary by lab and clinician. Alcohol can raise triglycerides for many people, so a result is easier to interpret when alcohol intake is stable or avoided for at least a day before testing.
The full triglyceride reference range is useful when comparing low, optimal, borderline, high, and very high results. For broader context, a standard lipid panel includes total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides, and those values should be read together rather than one by one.
Common Causes of Low Triglycerides
Low triglycerides can come from lower intake, lower absorption, lower liver production, faster clearance from the blood, medication effects, or inherited differences in lipoprotein metabolism. The most likely cause depends on whether the result is new or lifelong, mild or extreme, and whether other blood tests are normal.
Longer fasting or recent calorie restriction
A long fast can lower triglycerides because fewer dietary fats are entering the bloodstream and the liver may send out less VLDL. Someone who skipped dinner, fasted for 14–18 hours, had an early morning blood draw, or had a low-calorie day before testing may see a lower number than usual.
Short-term illness can have a similar effect. A stomach virus, poor appetite, fever, or several days of reduced food intake may temporarily lower triglycerides. In this situation, the result often returns toward the person’s usual range after normal eating resumes.
Low-carbohydrate or ketogenic diets
Low-carbohydrate diets often lower triglycerides, sometimes dramatically. Refined carbohydrates and sugars can increase liver production of triglycerides, especially in people with insulin resistance. When carbohydrate intake falls and weight improves, liver VLDL output often falls too. A triglyceride drop from 160 mg/dL to 60 mg/dL after major diet changes is common and may be a favorable metabolic sign.
Ketogenic diets can produce low triglycerides in some people, although LDL cholesterol can rise in others. That is why low triglycerides should not be interpreted as a complete cardiovascular risk assessment. LDL cholesterol, non-HDL cholesterol, ApoB, blood pressure, glucose markers, smoking status, family history, and personal cardiovascular history still matter.
Very-low-fat diets
A very-low-fat diet can also lower triglycerides, especially in the nonfasting state, because there is less dietary fat entering the bloodstream after meals. This does not always mean the diet is ideal. Fat is needed for essential fatty acids and absorption of vitamins A, D, E, and K. A diet that is extremely low in fat may cause problems if it is too restrictive, too low in calories, or poor in nutrient quality.
Lipid-lowering medications and supplements
Several treatments can lower triglycerides. These include fibrates, prescription omega-3 fatty acids, high-dose EPA products, niacin, statins, some PCSK9 inhibitors, and newer therapies that affect ApoC-III or ANGPTL3 pathways. Over-the-counter fish oil can lower triglycerides modestly in some people, but the effect varies by dose, product quality, baseline triglyceride level, and diet.
A low triglyceride result after starting medication is often expected. The more important question is whether the medication is still needed, whether LDL cholesterol and ApoB are at safe targets, and whether side effects are present. Medication should not be stopped just because triglycerides are low unless a clinician recommends it.
Hyperthyroidism
An overactive thyroid can lower cholesterol and triglycerides by speeding up metabolism and increasing lipid turnover. Clues may include unexplained weight loss, heat intolerance, sweating, tremor, anxiety, rapid heartbeat, frequent bowel movements, muscle weakness, or sleep difficulty. Thyroid-stimulating hormone, or TSH, is commonly used as the first screening test.
Malnutrition, malabsorption, and chronic digestive problems
Low triglycerides may reflect poor intake or poor absorption of fat. This becomes more likely when low triglycerides appear with weight loss, chronic diarrhea, greasy or floating stools, bloating, low albumin, anemia, low vitamin D, low vitamin E, or other nutrient deficiencies.
Possible causes include untreated celiac disease, inflammatory bowel disease, pancreatic insufficiency, bile acid problems, intestinal surgery, severe liver or bile duct disease, eating disorders, cancer, chronic infection, or other conditions that reduce intake or absorption. In these situations, the triglyceride number is not the main problem; it is a clue that the body may not be getting or absorbing enough nutrients.
Genetic low-lipid disorders
Some inherited conditions cause lifelong low total cholesterol, low LDL cholesterol, low ApoB, low VLDL, and low triglycerides. Familial hypobetalipoproteinemia, abetalipoproteinemia, and related disorders affect the assembly or secretion of ApoB-containing lipoproteins. Mild forms may be discovered by accident in adulthood. Severe forms can cause fat malabsorption, poor growth, neurologic problems, vision problems, acanthocytosis, fatty liver, and deficiencies of fat-soluble vitamins.
Genetic causes are more likely when triglycerides, LDL cholesterol, total cholesterol, and ApoB are all very low, especially if the pattern has been present for years or appears in family members.
Diet, Fasting, Exercise, and Weight Loss
Diet and lifestyle are the most common explanations for low or low-normal triglycerides in otherwise healthy adults. A single triglyceride result is a snapshot of recent metabolism, not a permanent label.
Regular exercise can lower triglycerides by improving muscle uptake of fatty acids and increasing insulin sensitivity. Aerobic activity, resistance training, and weight loss can all reduce triglyceride-rich particles. This is one reason active people often have triglycerides below 100 mg/dL. When HDL cholesterol is healthy and glucose markers look good, this pattern often suggests better metabolic flexibility.
Weight loss can lower triglycerides quickly, particularly when the starting level was high. A 5–10% reduction in body weight can produce meaningful triglyceride improvement in people with insulin resistance, fatty liver, or metabolic syndrome. When weight loss is intentional, gradual, and paired with adequate protein and nutrient intake, lower triglycerides are usually not a problem. When weight loss is unintentional, rapid, or accompanied by fatigue, pain, diarrhea, or loss of appetite, the same lab pattern deserves more medical attention.
Carbohydrate quality matters. Diets high in sugar, fruit juice, sweet drinks, refined grains, and frequent desserts tend to raise triglycerides in susceptible people. Replacing these with minimally processed foods often lowers triglycerides. A person who recently reduced sugar, alcohol, and refined starch may see triglycerides fall into the 40–70 mg/dL range.
Fat intake also matters, but not in a simple “less is always better” way. Extremely low-fat diets may lower triglycerides, yet they can make it harder to absorb fat-soluble vitamins and get enough essential fatty acids. A more balanced pattern usually includes unsaturated fats from foods such as olive oil, nuts, seeds, avocado, and fish, unless a medical condition requires a specific fat restriction.
Alcohol often raises triglycerides, especially when intake is heavy or paired with high-calorie meals. Stopping alcohol can lower triglycerides. In someone whose triglycerides were previously high, a low result after reducing alcohol may reflect a genuine improvement.
Intermittent fasting, prolonged fasting, and very low calorie diets can lower triglycerides, but they can also blur interpretation. A triglyceride result after a 20-hour fast is not the same as a result after a typical 8–12-hour fast. If the number is surprisingly low, repeating the test under standard conditions often clears up the confusion.
Health Effects and When Low Levels Matter
Low triglycerides are usually not harmful by themselves. In many people, lower triglycerides go along with lower remnant cholesterol, better insulin sensitivity, lower liver fat, and lower cardiometabolic risk. A triglyceride level of 55 mg/dL in a healthy adult with normal weight, normal glucose, normal thyroid function, normal liver tests, normal LDL cholesterol or ApoB, and no symptoms usually does not need treatment.
Low triglycerides become more meaningful when they are part of a larger abnormal pattern.
A very low triglyceride result can be a sign of undernutrition when it appears with low body weight, low albumin, low prealbumin, anemia, low lymphocyte count, low cholesterol, low vitamins, or muscle wasting. It can also be a sign of malabsorption when paired with chronic diarrhea, oily stools, bloating, or deficiencies in vitamins A, D, E, or K.
Severe inherited low-lipid disorders can affect the nervous system and eyes because vitamin E and other fat-soluble nutrients depend on lipid transport and absorption. Symptoms can include poor coordination, neuropathy, muscle weakness, night blindness, retinal disease, and abnormal red blood cell shapes. These disorders are rare, but they should be considered when lipid values are extremely low and persistent.
Low triglycerides can also appear during chronic illness. Advanced liver disease, cancer, chronic inflammatory disease, severe infection, and hyperthyroidism can lower lipid levels through reduced intake, altered metabolism, or increased energy use. In those cases, the low triglyceride result is a clue rather than the diagnosis.
Cardiovascular interpretation needs nuance. High triglycerides often travel with atherogenic remnant particles, low HDL cholesterol, insulin resistance, and small dense LDL patterns. Lower triglycerides generally suggest fewer triglyceride-rich particles, but they do not prove that artery risk is low. ApoB, LDL cholesterol, non-HDL cholesterol, Lp(a), blood pressure, smoking, diabetes, kidney function, family history, and existing plaque still shape risk. A person can have low triglycerides and high LDL cholesterol, high ApoB, or high Lp(a).
The triglyceride/HDL ratio can be helpful in metabolic screening, but very low triglycerides can make the ratio look excellent even when other risks exist. The triglyceride/HDL ratio is best used as one clue, not as a replacement for a full risk assessment.
Low triglycerides do not usually cause pancreatitis. Pancreatitis risk is linked mainly to severe high triglycerides, especially when levels rise into the hundreds or thousands. Low triglycerides also do not usually cause fatigue, dizziness, or weakness directly. When those symptoms occur, the cause is more likely to be calorie deficit, thyroid disease, anemia, infection, dehydration, medication effects, or another medical issue.
Related Blood Tests That Help Explain the Result
A low triglyceride result is easiest to interpret by looking at the rest of the lipid panel first. Total cholesterol, LDL cholesterol, HDL cholesterol, calculated VLDL cholesterol, and non-HDL cholesterol show whether the result is isolated or part of a broader low-lipid pattern.
If triglycerides are low but LDL cholesterol and ApoB are high, cardiovascular risk may still be elevated because ApoB-containing particles can still enter artery walls. If triglycerides, LDL cholesterol, VLDL cholesterol, total cholesterol, and ApoB are all very low, inherited hypobetalipoproteinemia or malabsorption becomes more plausible.
ApoB measures the number of major atherogenic lipoprotein particles, including LDL, VLDL remnants, and IDL. It can help clarify risk when standard cholesterol values are confusing. An ApoB blood test may be useful when triglycerides are low but LDL cholesterol is unusual, family history is strong, or a more particle-focused risk estimate is needed.
Glucose and insulin markers help explain whether low triglycerides reflect improved metabolic health. Fasting glucose, HbA1c, fasting insulin, and sometimes HOMA-IR can show whether insulin resistance is present. Low triglycerides with normal fasting insulin, normal glucose, normal waist size, and healthy blood pressure often points toward good metabolic health. Low triglycerides with high LDL cholesterol after a very-low-carb diet may need a different discussion. A broader metabolic syndrome blood test panel can help when weight, blood pressure, glucose, HDL cholesterol, and triglycerides need to be assessed together.
Thyroid testing is important when the result is new and unexplained. TSH, free T4, and sometimes free T3 can check for hyperthyroidism. Low TSH with high free T4 or free T3 supports an overactive thyroid pattern.
Liver and nutrition markers can help when low triglycerides occur with symptoms. A comprehensive metabolic panel may show albumin, liver enzymes, bilirubin, kidney markers, and electrolytes. A CMP blood test does not diagnose every cause, but it can reveal clues such as liver inflammation, low protein status, or kidney issues.
Iron, B12, folate, vitamin D, vitamin E, prothrombin time or INR, and other nutrient markers may be needed when malabsorption is suspected. Fat-soluble vitamin testing is especially relevant in severe genetic low-lipid disorders or chronic fat malabsorption.
Stool tests, celiac antibody testing, pancreatic elastase, inflammatory markers, and imaging may be considered when digestive symptoms are present. These are not routine follow-ups for every low triglyceride result, but they become useful when the story points toward poor absorption, chronic inflammation, or pancreatic disease.
What to Do After a Low Triglyceride Result
A low triglyceride result should be handled in proportion to the number, the pattern, and the person’s health. Most mild low results do not require treatment. The most useful first step is to confirm whether the result fits the situation.
If the level is between 50 and 100 mg/dL and the person feels well, no special action is usually needed. This range often reflects good metabolic health, especially if HDL cholesterol is healthy and glucose markers are normal.
If the level is below 50 mg/dL, review the circumstances before assuming disease. Ask whether the blood draw followed a longer-than-usual fast, recent illness, calorie restriction, heavy exercise, weight loss, alcohol reduction, a new diet, or a lipid-lowering medication. If one of these explains the result, the clinician may simply monitor it.
If the level is below 30 mg/dL, or if low values repeat, a more careful review is reasonable. The workup may include:
- Repeating a fasting lipid panel under standard conditions
- Comparing with older lipid results
- Reviewing prescription drugs, supplements, and recent diet changes
- Checking TSH and free T4 for hyperthyroidism
- Reviewing weight trend, appetite, bowel habits, and alcohol use
- Checking liver enzymes, albumin, CBC, glucose markers, and nutrient markers when appropriate
- Measuring ApoB if LDL cholesterol, total cholesterol, and VLDL cholesterol are also low
- Considering digestive or genetic evaluation when the pattern is extreme or lifelong
Do not try to raise triglycerides by adding sugar, refined carbohydrates, alcohol, or excess calories. That can worsen insulin resistance, fatty liver, blood pressure, weight, and cardiovascular risk. If low triglycerides are due to healthy diet and activity, raising them is not a goal.
If the cause is poor intake, the solution is better nutrition, not junk food. Meals should provide enough calories, protein, fiber-rich carbohydrates, and healthy fats. People who struggle to maintain weight may need energy-dense foods such as olive oil, nut butters, avocado, yogurt, eggs, legumes, whole grains, and fish, depending on tolerance and medical needs.
If malabsorption is suspected, treatment depends on the cause. Celiac disease, inflammatory bowel disease, pancreatic insufficiency, bile acid disorders, and liver or biliary disease require different care. Fat-soluble vitamin deficiencies may need targeted supplementation, but doses should be guided by testing and medical supervision, especially for vitamins A, D, E, and K.
If medication is the likely reason, the prescribing clinician can decide whether the dose still fits the person’s risk profile. A low triglyceride value alone is not usually a reason to stop a statin, prescription omega-3 product, fibrate, or other lipid therapy. Medication decisions should consider LDL cholesterol, ApoB, cardiovascular history, pancreatitis history, diabetes, kidney function, side effects, and treatment goals.
Children, pregnant people, and people with known genetic lipid disorders need more individualized interpretation. Pediatric lipid values vary by age, and very low lipid levels in children can be more concerning when paired with poor growth, chronic diarrhea, or neurologic symptoms. Pregnancy usually raises triglycerides, so unexpectedly low values during pregnancy should be interpreted with the full clinical picture.
Medical review is especially important when low triglycerides occur with unintentional weight loss, persistent diarrhea, greasy stools, fainting, severe fatigue, rapid heartbeat, tremor, heat intolerance, jaundice, swelling, easy bruising, night blindness, numbness, balance problems, or a strong family pattern of very low cholesterol.
