What a Low Vitamin B12 Result Means
A low vitamin B12 blood test means your circulating B12 level is below the reference range used by that lab. The result suggests your body may not have enough B12 available for red blood cell production, DNA synthesis, and nerve function.
Vitamin B12 is a water-soluble vitamin found naturally in animal foods such as meat, fish, eggs, and dairy. Some foods, especially cereals, plant milks, and nutritional yeast, may be fortified with B12. The body does not make its own B12, so intake and absorption both matter.
Absorption is the part that makes B12 different from many other vitamins. B12 in food must be released from protein in the stomach, then bind to intrinsic factor, a protein made by stomach parietal cells. The B12-intrinsic factor complex is absorbed near the end of the small intestine, in the ileum. A problem at any step can lower B12 even when the diet contains enough of it.
A low result can mean one of several things:
- Low intake: not enough B12 from food or fortified products.
- Poor release from food: often from low stomach acid, aging, or acid-suppressing medicines.

Vitamin B12 Ranges and Borderline Results
Vitamin B12 ranges vary by lab, country, testing method, and reporting unit. Many reports use picograms per milliliter, written as pg/mL. Some use picomoles per liter, written as pmol/L.
A common pattern is:

A person with a B12 of 180 pg/mL and numb feet needs a different level of concern than someone with a mild low result after one restricted diet period and no symptoms. A person with a “normal” B12 result but classic neurologic symptoms may still need further evaluation, especially if MMA is high.
The serum B12 test measures B12 circulating in the blood. It includes B12 bound to different carrier proteins, not all of which reflects active B12 delivery into cells. That is why a borderline result can be hard to interpret.
Two practical questions help:
Are there signs the body is struggling to use B12?
A high MMA level supports B12 deficiency because MMA rises when B12-dependent metabolism slows. Homocysteine can also rise, but it is less specific because folate deficiency, kidney function, thyroid disease, and other factors can affect it.
Is there a reason the result might become low again?
A short-term diet change may be easier to correct than autoimmune gastritis, ileal surgery, or long-term metformin use. Persistent causes often need ongoing replacement and periodic follow-up.
The unit conversion is also worth noting. A B12 level around 200 pg/mL is roughly 148 pmol/L, and 250 pg/mL is roughly 185 pmol/L. Exact interpretation should still follow the lab report and clinician’s judgment.
Common Causes of Low Vitamin B12
Low vitamin B12 usually comes from reduced intake, reduced absorption, or both. In adults, absorption problems are very common, especially when B12 stays low despite a reasonable diet.
Low dietary intake
Vitamin B12 is naturally found in animal foods. People who eat no meat, fish, eggs, or dairy are at higher risk unless they regularly use fortified foods or B12 supplements. Vegetarian diets can also be low in B12 if eggs and dairy are limited or inconsistent.
Low intake may also happen with poor appetite, eating disorders, alcohol use disorder, food insecurity, or very limited diets. Older adults may eat less protein-rich food and may also absorb B12 less efficiently, creating a double risk.
Plant foods do not reliably provide active B12 unless they are fortified. Seaweed, spirulina, and fermented foods are sometimes promoted as B12 sources, but they should not be relied on to correct or prevent deficiency.
Autoimmune gastritis and intrinsic factor problems
Autoimmune gastritis damages the stomach cells that make intrinsic factor. Without enough intrinsic factor, the body cannot absorb B12 normally through the usual pathway.
This condition has long been associated with the term pernicious anemia. Strictly speaking, pernicious anemia refers to the anemia caused by autoimmune B12 malabsorption. A person can have autoimmune B12 deficiency before anemia develops.
Clues can include a very low B12 level, recurrent deficiency after stopping treatment, a family or personal history of autoimmune disease, iron deficiency, and positive intrinsic factor or parietal cell antibodies. Autoimmune gastritis may also increase the need for long-term monitoring beyond simply replacing B12.
Stomach acid problems and acid-suppressing medicines
Stomach acid helps release B12 from food protein. Lower acid levels can reduce absorption of food-bound B12. This can happen with aging, chronic gastritis, and long-term use of proton pump inhibitors or H2 blockers.
This does not mean everyone using acid-suppressing medicine will become deficient. Risk rises when several factors combine, such as older age, low dietary intake, long duration of medication use, or other gastrointestinal disease.
Metformin
Metformin can lower vitamin B12 levels, especially with higher doses, longer use, and other risk factors. This matters because B12 deficiency can also cause neuropathy, and neuropathy may already be a concern in diabetes.
People taking metformin should not stop it on their own because of a low B12 result. The usual approach is to test when symptoms or risk factors are present, correct B12 deficiency when found, and continue diabetes treatment unless the prescribing clinician recommends otherwise.
Gastrointestinal disease or surgery
B12 absorption depends on the stomach, pancreas, and ileum working together. Low B12 can occur after bariatric surgery, gastrectomy, ileal resection, or other surgery that changes normal digestion and absorption.
Crohn’s disease affecting the ileum can reduce B12 absorption. Celiac disease, pancreatic insufficiency, chronic inflammatory bowel disease, bacterial overgrowth, and some infections can also contribute, depending on the person.
Mixed deficiencies
B12 deficiency can occur with iron deficiency or folate deficiency. Mixed patterns can make blood count results look less obvious. For example, iron deficiency tends to make red blood cells smaller, while B12 deficiency tends to make them larger. Together, the MCV may look closer to normal even when both problems are present.
This is one reason a complete anemia evaluation often includes B12, folate, ferritin, iron studies, and a CBC pattern rather than one marker alone. Symptoms, Anemia, and Nerve Effects
Low vitamin B12 can affect the blood, nerves, mouth, mood, cognition, and digestive tract. Symptoms may be mild, vague, or absent early on. A person can also have nerve symptoms without obvious anemia.
Anemia symptoms
B12 is needed for normal DNA production in developing red blood cells. When deficiency interferes with this process, red blood cells may become large and immature. This is called megaloblastic anemia and often appears as a high MCV on a CBC.
Common anemia-related symptoms include:
- unusual tiredness or weakness
- shortness of breath with activity
- dizziness or lightheadedness
- paleness
- fast heartbeat or palpitations
- reduced exercise tolerance
- headaches

Nerve and brain symptoms
Vitamin B12 supports myelin, the protective covering around nerves. Low B12 can cause neurologic symptoms that may become harder to reverse if treatment is delayed.
Possible nerve and brain symptoms include:
- numbness, tingling, or burning in the feet or hands
- reduced vibration or position sense
- balance problems or unsteady walking
- weakness
- clumsiness
- memory changes
- confusion
- mood changes, irritability, or depression
- vision changes in severe cases
These symptoms should be taken seriously even if the CBC looks normal. Anemia is not required for B12-related nerve injury. Some people first notice tingling toes, poor balance in the dark, or a “walking on cotton” feeling rather than fatigue.
Mouth, digestive, and skin clues
B12 deficiency can also cause a sore, red, or smooth tongue; mouth ulcers; appetite loss; nausea; diarrhea; constipation; or weight loss. Some people notice pale or slightly yellow skin if anemia or increased red cell breakdown is present.
These symptoms overlap with many other conditions, so they do not diagnose B12 deficiency by themselves. They are useful clues when they appear alongside a low result, risk factors, or an abnormal CBC.
Why symptoms may not match the number
The B12 number and symptoms do not always move together. A person with a very low result may feel surprisingly well. Another person with a borderline result may have symptoms because cellular B12 use is impaired or the level has been low long enough to affect nerves.
Several factors can blur the picture:
- folate intake can improve anemia while nerve problems continue
- iron deficiency can hide the expected high MCV
- kidney disease can raise MMA even without simple B12 deficiency
- recent supplements can raise serum B12 before symptoms recover
- neurologic recovery often lags behind blood marker improvement
This is why B12 deficiency is best interpreted as a clinical pattern, not just a single number.
Follow-Up Tests That Clarify Low B12
Follow-up testing helps confirm whether low B12 is affecting the body, identify the cause, and check for related deficiencies. The right tests depend on the result, symptoms, diet, and medical history.

A complete blood count is often one of the first companion tests because it shows whether low B12 is affecting blood cell production. MCV and RDW can help identify macrocytosis or mixed anemia patterns, but normal results do not rule out B12-related nerve symptoms.
MMA is often the most helpful confirmatory test. When B12 is borderline and MMA is high, functional B12 deficiency becomes more likely. The methylmalonic acid test is especially useful when symptoms, blood counts, and serum B12 do not line up neatly.
Homocysteine can add information, particularly when folate status is also being considered. High homocysteine can occur with low B12, low folate, kidney disease, hypothyroidism, and some medications. Paired interpretation is often clearer than either marker alone, especially when using homocysteine and MMA to separate B12 and folate patterns.
Cause testing is important when deficiency is severe, recurrent, unexplained, or likely to require long-term treatment. Intrinsic factor antibody testing is commonly used when autoimmune B12 malabsorption is suspected. Gastrointestinal evaluation may be needed when symptoms suggest Crohn’s disease, celiac disease, malabsorption, chronic gastritis, or complications after surgery.
Testing after supplementation can be tricky. Serum B12 may rise quickly after tablets or injections, but nerve symptoms can take months to improve. A high B12 result after treatment does not always mean the original diagnosis was wrong. It may simply show that replacement is reaching the blood.
Treatment and How Levels Recover
Treatment replaces vitamin B12 and addresses the cause when possible. The route, dose, and duration depend on severity, symptoms, absorption, and whether the cause is temporary or ongoing.
Oral B12
High-dose oral B12 can work for many people, including some with absorption problems, because a small amount can be absorbed by passive diffusion even without perfect intrinsic factor function. Common high-dose oral regimens often use 1,000 mcg daily at first, but exact dosing should come from a clinician.
Oral treatment may be reasonable when symptoms are mild, there are no serious neurologic signs, adherence is reliable, and follow-up testing is available. It is also often used for dietary deficiency, long-term prevention in vegan diets, and maintenance after levels recover.
B12 injections
Injections are often used when deficiency is severe, neurologic symptoms are present, absorption is clearly impaired, or rapid correction is needed. They may also be used when a person cannot reliably absorb or take oral supplements.
Injection schedules vary. Some protocols start with frequent injections over days or weeks, then move to maintenance injections every 1 to 3 months. Hydroxocobalamin and cyanocobalamin are common forms, with availability differing by country.
Injections are not automatically better for every person with low B12, but they are often preferred when symptoms are significant or the risk of irreversible nerve injury is a concern.
Treating the cause
Replacing B12 corrects the deficiency, but preventing recurrence depends on the cause.
- Low intake: regular B12 supplements or fortified foods are usually needed if the diet remains low in animal products.
- Autoimmune gastritis: long-term or lifelong replacement is usually needed because intrinsic factor production does not reliably return.
- Bariatric or stomach surgery: scheduled supplementation and monitoring are often part of long-term care.
- Metformin: B12 replacement can usually be added while metformin continues.
- Acid suppression: the clinician may reassess dose and duration, but acid-suppressing medicine should not be stopped without medical advice when it is needed.
- Crohn’s disease or ileal disease: the bowel condition and B12 deficiency both need attention.
How fast recovery happens
Blood response often starts before symptom response. Reticulocytes, which are young red blood cells, may rise within about a week after effective treatment. Hemoglobin and MCV often improve over weeks, though full normalization can take longer depending on severity and whether iron or folate deficiency is also present.
Nerve recovery is less predictable. Tingling and balance symptoms may improve over months, but long-standing symptoms can persist. Earlier treatment improves the chance of better recovery.
A practical follow-up plan may include:
- Confirm the diagnosis and likely cause.
- Start replacement at an appropriate dose and route.
- Recheck blood counts and symptoms after the early treatment period.
- Check MMA or other markers if the response is unclear.
- Decide whether maintenance treatment is temporary, long-term, or lifelong.
- Monitor for recurrence in people with ongoing risk factors.
Do not treat low B12 by taking high-dose folic acid alone. Folate can improve the anemia pattern while B12-related nerve injury continues. A combined supplement may be appropriate in some cases, but known or suspected B12 deficiency should be corrected directly.
Prevention, Monitoring, and When to Get Care
Preventing another low B12 result depends on why it became low. Some people need only a short course and a diet change. Others need lifelong replacement.
Food and supplement prevention
Adults generally need 2.4 mcg of vitamin B12 per day, with higher recommended intake during pregnancy and lactation. This daily requirement is small, but absorption is limited, so supplements often contain much larger amounts than the daily requirement.
Good food sources include fish, shellfish, meat, poultry, eggs, milk, yogurt, and cheese. Fortified nutritional yeast, fortified cereals, and fortified plant milks can help people who avoid animal foods. The label matters because not every plant milk, cereal, or nutritional yeast product contains B12.
People eating vegan diets usually need a reliable B12 supplement or consistent fortified foods. Occasional intake is not enough for everyone, and symptoms can take a long time to appear.
Who may need periodic testing
Periodic B12 testing may be useful for people with:
- vegan or very low-animal-food diets
- prior low B12
- metformin use, especially long-term or high-dose use
- long-term proton pump inhibitor or H2 blocker use with other risk factors
- bariatric surgery, gastrectomy, or ileal surgery
- Crohn’s disease affecting the ileum
- autoimmune gastritis or positive intrinsic factor antibodies
- unexplained macrocytosis
- neuropathy, balance symptoms, or cognitive changes without a clear cause
- older age plus poor intake or digestive problems
The testing interval varies. Someone with a new mild dietary deficiency may need short-term follow-up after treatment. Someone with autoimmune gastritis or bariatric surgery may need a long-term schedule.
When medical care should be prompt
Low B12 should be discussed with a healthcare professional, but some symptoms need faster attention. Seek prompt medical care for new or worsening numbness, weakness, trouble walking, falls, confusion, vision changes, severe fatigue, chest pain, fainting, or shortness of breath.
Urgent evaluation is also important when anemia is severe, when multiple blood cell lines are low, or when neurologic symptoms are progressing. These symptoms can come from B12 deficiency, but they can also come from other serious conditions.
Pregnancy, breastfeeding, infancy, and childhood deserve special caution. B12 deficiency during these periods can affect development and should be handled with medical guidance.
Low B12 is usually treatable. The most important step is not just raising the lab number, but understanding why it became low, whether the body shows signs of deficiency, and what plan will keep levels adequate over time.
