
Alanine aminotransferase, usually shortened to ALT, is a liver enzyme measured on many routine blood panels. ALT helps liver cells process amino acids, but only a small amount normally circulates in the blood. When liver cells become irritated, inflamed, injured, or more “leaky,” ALT can rise before a person feels sick. That makes ALT useful, but it also makes it easy to overinterpret. A mildly high ALT does not automatically mean serious liver disease, and a normal ALT does not always prove the liver is perfectly healthy.
The normal ALT range depends on the lab, the testing method, age, sex, body size, and the health of the group used to build the reference interval. Many lab reports list an adult ALT range around 7–56 U/L, while liver specialists often use lower “healthy” upper limits, especially when screening for fatty liver, hepatitis, alcohol-related injury, medication effects, and metabolic risk.

What ALT Measures
ALT is an enzyme found mostly inside liver cells. Enzymes are proteins that help chemical reactions happen. ALT helps move amino groups between molecules during amino acid metabolism, which is part of how the body handles protein and energy. A small amount of ALT normally leaks into the bloodstream as cells turn over.
ALT is often described as a “liver enzyme,” but it is not a liver function test in the strictest sense. It does not measure how well the liver makes proteins, clears bilirubin, produces bile, or supports blood clotting. Instead, ALT is mainly a marker of liver cell injury or irritation. When hepatocytes, the main working liver cells, are inflamed or damaged, more ALT can enter the blood.
ALT is commonly ordered as part of a comprehensive metabolic panel, hepatic function panel, or broader liver function tests panel. It may also be checked by itself when a clinician is monitoring known liver disease, medication safety, viral hepatitis, fatty liver disease, alcohol-related liver injury, or abnormal results from a previous test.
ALT is more liver-focused than AST, another aminotransferase enzyme. AST is found in the liver too, but it is also present in meaningful amounts in skeletal muscle, heart, red blood cells, kidneys, and brain. ALT results are usually reported in units per liter, written as U/L or IU/L. For everyday interpretation, U/L and IU/L are used similarly on most lab reports. The number should always be interpreted with the reference range printed next to it, because laboratories do not all use the same methods or limits.
ALT Normal Range and Healthy Upper Limits
A typical adult ALT reference range is often listed around 7–56 U/L, but this is not universal. Some laboratories use lower upper limits, such as about 35–45 U/L. Others use sex-specific ranges, because ALT values tend to run higher in adult men than in adult women.
A reference range is not the same as an ideal health target. Many lab reference ranges are built from a population sample and often include people with unrecognized fatty liver, insulin resistance, alcohol use, viral hepatitis risk, or other conditions that can nudge ALT upward. This is one reason liver specialists often talk about a lower “healthy” ALT range.

Several liver guidelines and studies suggest that “healthy” ALT upper limits are closer to 29–33 U/L for men and 19–25 U/L for women. These are not always printed on routine lab reports, but they help explain why a clinician may take an ALT of 42 U/L seriously even if the report marks it as “normal.”
For example, a woman with an ALT of 44 U/L may be inside one laboratory’s printed range but above many liver-health cutoffs. If she also has high triglycerides, insulin resistance, central weight gain, or ultrasound evidence of fatty liver, that ALT may deserve follow-up. A man with an ALT of 52 U/L may also be technically “normal” in a lab using 56 U/L as the upper limit, but the result could still fit early fatty liver, alcohol-related irritation, medication effect, or another mild liver injury pattern.
Children and adolescents need age-specific interpretation. ALT ranges differ in infants, children, and teens, and pediatric fatty liver guidance often uses lower thresholds than many adult lab reports. Pregnancy can also change the way liver tests are interpreted, because certain liver and bile duct conditions matter more urgently during pregnancy.
The most useful interpretation comes from the trend. A single ALT of 47 U/L may be less concerning if it returns to 22 U/L on repeat testing after avoiding alcohol, intense exercise, and unnecessary supplements. The same number matters more if it persists for months, rises over time, or appears with abnormal AST, GGT, bilirubin, alkaline phosphatase, albumin, INR, or platelet count.
What High ALT Can Mean
High ALT usually means liver cells are irritated, inflamed, or injured, but the number alone does not identify the cause. ALT can rise from common, mild, and reversible problems. It can also rise from conditions that need urgent care. The result is a clue, not a diagnosis.
A mild ALT elevation is often found during routine testing in people who feel well. Common causes include metabolic dysfunction-associated steatotic liver disease, often still called fatty liver disease; alcohol use; recent intense exercise; viral hepatitis; medication effects; supplement-related liver injury; and weight gain or insulin resistance. People with high ALT may have no pain, no jaundice, and no obvious digestive symptoms.
Fatty liver is one of the most common reasons for a persistent mild ALT elevation. It is more likely when ALT is high along with abdominal weight gain, high triglycerides, low HDL cholesterol, high fasting glucose, high fasting insulin, prediabetes, type 2 diabetes, or high blood pressure. ALT may improve with weight loss, better insulin sensitivity, less alcohol, and treatment of metabolic risk factors, but a normal ALT does not rule out fatty liver or even fibrosis in every person.
Alcohol can raise ALT, but alcohol-related liver injury often raises AST more than ALT. A pattern with AST higher than ALT, especially when AST is roughly twice ALT and GGT is also high, may point toward alcohol-related liver stress. That said, no enzyme pattern can prove how much someone drinks. History, repeat testing, GGT, bilirubin, INR, albumin, platelets, imaging, and clinical context matter. Medications and supplements are another major category. Acetaminophen overdose can cause dramatic ALT and AST elevations and is a medical emergency. Some antibiotics, anti-seizure medicines, tuberculosis drugs, antifungals, bodybuilding supplements, green tea extract products, and multi-ingredient “detox” or weight-loss supplements can also injure the liver. Statins can mildly affect liver enzymes in some people, but they are often safe and beneficial when used appropriately; they should not be stopped without medical advice.
Viral hepatitis can raise ALT from mild to very high levels. Hepatitis A often causes an acute illness. Hepatitis B and C can be silent for years. A person can have chronic viral hepatitis with fluctuating liver enzymes, so screening depends on risk factors, age-based recommendations, exposure history, pregnancy status, and local public health guidance.
Autoimmune hepatitis, hemochromatosis, Wilson disease, alpha-1 antitrypsin deficiency, celiac disease, thyroid disease, and muscle disorders can also appear during the workup for high ALT. These are less common than fatty liver or alcohol-related causes, but they matter when ALT stays high, the patient is young, family history is suggestive, or other test results do not fit the common patterns.
Very high ALT, especially hundreds to thousands of U/L, narrows the possibilities. Acute viral hepatitis, drug-induced liver injury, toxin exposure, severe ischemic injury from low blood flow, and severe autoimmune hepatitis become more important. These situations need faster evaluation, especially if the person has yellow eyes, dark urine, pale stools, severe nausea, right upper abdominal pain, confusion, easy bleeding, severe weakness, or abnormal bilirubin or INR.
What Low ALT Can Mean
Low ALT is usually not interpreted with the same urgency as high ALT. Many healthy people have ALT values near the lower end of the range. A result such as 8 U/L or 10 U/L can be normal, especially if other blood tests are stable and there are no concerning symptoms.
Very low ALT may sometimes reflect low muscle mass, aging, frailty, poor nutrition, vitamin B6 deficiency, or chronic illness. ALT depends on pyridoxal-5-phosphate, the active form of vitamin B6, as a cofactor. When vitamin B6 status is low, measured aminotransferase activity can be lower than expected. This does not mean everyone with low ALT needs vitamin B6 supplements, but it gives clinicians a reason to interpret extremely low values in context.
Low ALT may also be seen in advanced chronic kidney disease, severe frailty, or states where the body has less muscle and liver reserve. In older adults, very low ALT has been studied as a possible marker of frailty rather than a direct liver diagnosis. It is usually more helpful to look at the whole picture: weight change, appetite, albumin, total protein, kidney function, inflammation markers, blood count, medications, and functional status.
A low ALT does not “cancel out” other abnormal liver findings. For example, a person with cirrhosis may have only mildly abnormal or even normal ALT because advanced scarred liver tissue may contain fewer healthy cells capable of leaking large amounts of enzyme. In that setting, platelet count, albumin, INR, bilirubin, imaging, spleen size, and fibrosis assessment can be more informative than ALT alone.
If ALT is low but the person feels well and other results are normal, follow-up may simply mean routine monitoring. If ALT is low along with weight loss, weakness, malnutrition, anemia, low albumin, abnormal kidney function, or neurological symptoms, the next step is not to treat the ALT number. It is to look for the underlying reason.

How ALT Fits With Other Liver Tests
ALT becomes much more useful when it is interpreted with the rest of the liver panel. A liver panel often includes AST, alkaline phosphatase, bilirubin, albumin, and sometimes GGT, total protein, direct bilirubin, and INR. These tests answer different questions.
ALT and AST mainly reflect liver cell injury. Alkaline phosphatase and GGT help identify bile duct or cholestatic patterns. Bilirubin reflects bile processing and excretion, as well as red blood cell breakdown in some cases. Albumin and INR help assess liver synthetic function, meaning the liver’s ability to make important proteins and clotting factors.
A high ALT with a similar or lower AST often fits a hepatocellular pattern. Common causes include fatty liver, viral hepatitis, autoimmune hepatitis, medication injury, and many toxin-related injuries. A high alkaline phosphatase with high GGT may point more toward bile duct obstruction, cholangitis, primary biliary cholangitis, primary sclerosing cholangitis, infiltrative liver disease, or drug-induced cholestasis. A high alkaline phosphatase with normal GGT may push the workup toward bone or other non-liver sources.
GGT is useful when ALT is borderline and the clinician is trying to understand whether alcohol, fatty liver, bile duct irritation, or medication effects may be involved. GGT is sensitive but not specific. It can rise with alcohol use, fatty liver, cholestasis, many medications, and other liver conditions. A combined GGT and ALT pattern often tells more than either result alone.
Bilirubin changes the level of concern. Mild ALT elevation with normal bilirubin and normal INR is often evaluated step by step. ALT elevation with jaundice, dark urine, or high direct bilirubin deserves more urgent attention. ALT or AST more than three times the upper limit of normal together with a bilirubin more than twice the upper limit can raise concern for more serious drug-induced or acute liver injury, depending on the clinical context.
Albumin and INR help separate liver irritation from impaired liver function. A person can have high ALT while the liver still performs its core jobs well. Low albumin or a high INR may suggest reduced synthetic function, severe acute liver injury, advanced chronic liver disease, vitamin K problems, blood thinner use, kidney protein loss, inflammation, or malnutrition. These results need careful interpretation rather than quick assumptions.
Platelet count can also matter. Low platelets can occur for many reasons, but in chronic liver disease they may suggest portal hypertension or advanced fibrosis. If ALT has been mildly high for years and platelets are trending low, a clinician may calculate fibrosis scores such as FIB-4 or APRI. Factors That Can Change ALT Results
ALT is not fixed. It can move from week to week, and sometimes from day to day, because liver cells respond to recent behavior, illness, medications, and metabolic stress.
Strenuous exercise can raise ALT and AST, especially if it causes muscle soreness or injury. AST often rises more than ALT in muscle injury, and creatine kinase may be high. Heavy weightlifting, long-distance running, intense interval training, or returning to exercise after a long break can affect results. If a person feels well and the timing fits, a clinician may repeat testing after several days to a week of avoiding unusually hard workouts.
Alcohol can raise ALT, AST, and GGT. The effect depends on amount, pattern, genetics, nutrition, body weight, other liver risks, and medication use. Binge drinking before a blood test can distort results. Regular heavy drinking can cause a more persistent pattern, sometimes with AST higher than ALT. Stopping alcohol may improve enzymes, but people who drink heavily every day should ask a clinician about safe withdrawal support rather than stopping abruptly on their own.
Body weight and insulin resistance strongly affect ALT. ALT can rise with liver fat accumulation even when a person does not drink much alcohol. Weight loss of about 5% can reduce liver fat in many people, and larger sustained losses may improve inflammation and scarring risk in selected patients. Rapid crash dieting, however, is not a good liver strategy and may worsen gallstone risk or be hard to sustain.
Food timing usually has less effect on ALT than on glucose or triglycerides. Many ALT tests do not require fasting. However, if ALT is drawn as part of a metabolic panel, lipid panel, fasting insulin test, or other fasting labs, the clinician may ask for 8–12 hours without food. The instructions depend on the full panel, not ALT alone.
Medications and supplements deserve a careful list. “Natural” does not mean liver-safe. Green tea extract, high-dose vitamin A, kava, certain bodybuilding supplements, anabolic steroids, concentrated herbal blends, and multi-ingredient weight-loss products have all been linked to liver injury. Prescription drugs can also matter, but the answer is not to stop everything. The safer approach is to review dose, timing, alcohol use, acetaminophen exposure, and alternatives with a clinician.
Short-term infections can also raise ALT. Viral illnesses, mononucleosis, COVID-19, sepsis, and other inflammatory states may affect liver enzymes. ALT may normalize after recovery, but follow-up is important if the elevation is large, persistent, or paired with jaundice or abnormal clotting tests.
Lab variation is another reason not to judge a person’s liver from one number. A repeat ALT at the same lab is often more comparable than a repeat at a different lab. If a result does not fit the person’s history or symptoms, repeating the test is common and reasonable.
Follow-Up and Next Steps
The right follow-up depends on the ALT level, symptoms, other liver tests, medical history, and how long the result has been abnormal. A mildly high ALT in a person who feels well is usually handled differently from a marked ALT elevation with jaundice or confusion.
For a mild, unexpected ALT elevation, clinicians often repeat the liver panel after a short interval. Before the repeat test, they may suggest avoiding alcohol, intense exercise, and unnecessary supplements. They may also review acetaminophen use, prescription medicines, recent illnesses, body weight changes, and metabolic risk factors.
If ALT remains high, the next step is usually a focused evaluation. This may include hepatitis B and C testing, iron studies, fasting glucose or A1c, lipid testing, autoimmune markers, thyroid testing, celiac screening, creatine kinase, ultrasound, or fibrosis scoring. The exact workup depends on the pattern. For example, high ALT with high triglycerides and insulin resistance points in a different direction than high ALT with jaundice, itching, and high alkaline phosphatase.
Persistent ALT elevation should not be ignored just because the person feels well. Many liver diseases are quiet in early stages. Fatty liver, chronic hepatitis B, chronic hepatitis C, autoimmune hepatitis, hemochromatosis, and medication-related injury can all appear before obvious symptoms. Early recognition gives people more options to prevent fibrosis, cirrhosis, and liver failure.
Urgent medical care is more appropriate when ALT is high along with warning signs. These include yellowing of the skin or eyes, dark tea-colored urine, pale stools, severe right upper abdominal pain, persistent vomiting, fever with worsening illness, confusion, fainting, severe weakness, easy bruising or bleeding, black stools, swelling of the abdomen, or a known overdose or toxin exposure. A high INR, high bilirubin, low blood sugar, or mental status change can signal serious liver dysfunction.
People with known liver disease may need a different threshold for action. Someone with cirrhosis, chronic hepatitis, autoimmune liver disease, liver transplant history, or chemotherapy exposure should follow the monitoring plan set by their clinician. A small enzyme change may matter more in a high-risk setting than it would in a low-risk routine screening result.
Lifestyle steps can help when the pattern fits fatty liver or metabolic risk. Sustainable weight loss when needed, regular physical activity, better sleep, diabetes control, triglyceride improvement, and limiting or avoiding alcohol can lower liver stress. Coffee consumption has been associated with lower risk of some liver outcomes in observational studies, but it should not be used as a substitute for diagnosis or treatment.
The most useful way to read ALT is to ask four plain questions: Is it truly above the lab range or above a stricter healthy range? Is it new, persistent, rising, or improving? Are AST, GGT, bilirubin, alkaline phosphatase, albumin, INR, and platelets normal? Are there symptoms, medications, alcohol exposure, metabolic risks, viral risks, or family history that explain the pattern?
ALT is a helpful early signal when it is used carefully. It works best as part of a pattern, a trend, and a clinical story—not as a standalone verdict on liver health.
