
ALT and AST are liver enzymes that often rise when liver cells are irritated, inflamed, or injured. They are useful warning lights, but they are not complete measures of liver function by themselves. A mildly high ALT after weight gain, a new medication, alcohol use, a viral illness, or hard exercise can mean something very different from a sudden ALT and AST in the hundreds or thousands. The pattern also matters: ALT often points more toward liver-cell injury, while AST can also come from muscle, heart, and other tissues.
The safest way to read these results is to combine the numbers with symptoms, timing, other liver tests, medications, alcohol intake, metabolic risk factors, and repeat testing when appropriate. Many abnormal results are manageable, but some need prompt evaluation. A calm, structured approach helps you avoid both panic and false reassurance.
- ALT and AST measure liver-cell injury, not liver function. Albumin, INR, bilirubin, and platelets help show how well the liver is working.
- Mild elevations are common. Results less than about 2–3 times the lab’s upper limit often need context and repeat testing rather than emergency action.
- Very high ALT or AST can be urgent. Levels in the hundreds or thousands can occur with acute hepatitis, drug toxicity, blocked blood flow, or severe injury.
- AST is less liver-specific than ALT. Heavy exercise, muscle injury, and rhabdomyolysis can raise AST and sometimes ALT.
- The AST/ALT ratio can help, but it cannot diagnose the cause alone. Alcohol-related injury, cirrhosis, and muscle sources can all change the pattern.
- Follow-up depends on the whole panel. Bilirubin, ALP, GGT, albumin, INR, platelet count, viral hepatitis tests, iron studies, and imaging may matter.

What ALT and AST Measure
ALT and AST are enzymes involved in amino acid metabolism. They normally live mostly inside cells. When cells are stressed or damaged, some of these enzymes leak into the bloodstream, where they can be measured on a blood test.
ALT stands for alanine aminotransferase. It is found in highest concentration in the liver, so a high ALT is often treated as a more liver-focused signal. AST stands for aspartate aminotransferase. AST is found in the liver too, but it is also present in skeletal muscle, heart muscle, red blood cells, kidneys, brain, and other tissues. That is why AST can rise from non-liver problems, especially muscle injury.
ALT and AST are often included in a comprehensive metabolic panel or a hepatic panel. A liver function tests panel usually includes other markers such as alkaline phosphatase, bilirubin, albumin, and sometimes GGT. Those related markers are often just as important as ALT and AST.
Despite the phrase “liver function tests,” ALT and AST do not directly show how well the liver is performing its major jobs. They mostly show liver-cell irritation or injury. A person can have high ALT and AST while the liver is still functioning well. A person with advanced cirrhosis can also have normal or only mildly abnormal ALT and AST because fewer active liver cells remain to leak enzymes.
A useful way to think about ALT and AST is this:
- ALT and AST answer: Are liver cells, muscle cells, or other tissues leaking enzymes?
- Albumin and INR answer: Is the liver making enough proteins and clotting factors?
- Bilirubin answers: Is the liver processing and clearing bile pigments normally?
- ALP and GGT answer: Is there a bile duct or cholestatic pattern?
- Platelets and fibrosis scores answer: Is there possible long-term scarring or portal pressure?
ALT and AST are important, but they are never the whole story.
Normal Ranges and How High Is High
ALT and AST ranges vary by laboratory, age, sex, body size, and testing method. Many lab reports list ALT somewhere around 7–56 U/L and AST around 10–40 U/L, but some expert groups use lower “healthy” upper limits, especially for ALT. For example, an ALT in the 40s may appear normal on one lab report but still be higher than expected for a lean adult without metabolic risk factors.
Always compare your result with the reference range printed on your own report. Then look at how far above that range the result is.

A small elevation is not automatically dangerous, but it should not be ignored if it persists. A single ALT of 55 U/L may simply be a transient bump. The same ALT staying elevated for months, especially with obesity, diabetes, high triglycerides, heavy alcohol use, or abnormal imaging, deserves follow-up.
Very high levels need a different mindset. ALT or AST above 500 U/L is not the same problem as ALT of 55 U/L. Levels above 1,000 U/L narrow the possibilities and often raise concern for acute viral hepatitis, acetaminophen injury, ischemic hepatitis from poor blood flow, or another major liver insult. In those situations, symptoms and liver function markers become especially important.
Low ALT and AST usually do not signal liver disease. Very low ALT may be seen in some people with low muscle mass or vitamin B6 deficiency, but low results are rarely the main issue in routine liver interpretation.
Patterns That Help Narrow the Cause
ALT and AST become more useful when you look at the pattern instead of one number. Pattern recognition does not replace medical evaluation, but it helps decide which possibilities deserve attention first.
ALT higher than AST
ALT higher than AST is common in many liver-cell injury patterns. Metabolic dysfunction-associated steatotic liver disease, often still called fatty liver, commonly causes a mild to moderate ALT-predominant pattern. Viral hepatitis, autoimmune hepatitis, medication reactions, and many other liver conditions can also raise ALT more than AST.
In everyday practice, a mildly high ALT with normal or mildly high AST often leads to questions about weight changes, waist size, insulin resistance, triglycerides, alcohol intake, medications, supplements, viral hepatitis risk, and ultrasound findings. If metabolic risk is part of the picture, GGT and ALT together can provide a wider view of fatty liver, alcohol-related stress, and bile duct irritation.
AST higher than ALT
AST higher than ALT can happen for several reasons. Alcohol-related liver injury often raises AST more than ALT, commonly with an AST/ALT ratio above 2, although this pattern is not perfect. Cirrhosis from many causes can also make AST exceed ALT. Muscle injury is another important possibility, especially if AST is clearly high after intense workouts, seizures, trauma, injections, or muscle pain.
The difference between ALT and AST matters because AST is not liver-specific. If AST is high and ALT is normal or only slightly high, checking creatine kinase can help determine whether muscle is contributing.
Both ALT and AST high, but ALP only mildly changed
This is usually called a hepatocellular pattern, meaning the main signal points toward liver-cell injury rather than bile duct blockage. Common causes include fatty liver, viral hepatitis, alcohol-related liver injury, autoimmune hepatitis, drug-induced liver injury, ischemic injury, and toxin exposure.
The degree of elevation matters. Mild hepatocellular patterns are common and often chronic. Sudden marked elevations need faster attention.
ALT and AST high with ALP and bilirubin also high
A mixed pattern can occur when liver-cell injury and bile flow problems overlap. Gallstones, bile duct obstruction, drug reactions, severe hepatitis, and advanced liver disease can produce mixed results. In that setting, comparing ALP and GGT patterns can help separate liver or bile duct sources from bone-related ALP elevation.
Common Causes of High ALT and AST
High ALT and AST have a long list of possible causes. The most likely explanation depends on the size of the elevation, how long it has been present, symptoms, risk factors, medications, and other test results.
Metabolic fatty liver
Metabolic fatty liver is one of the most common reasons for mild ALT and AST elevation. It is linked with insulin resistance, type 2 diabetes, abdominal weight gain, high triglycerides, low HDL cholesterol, high blood pressure, and sleep apnea. Some people have normal liver enzymes despite significant fatty liver, while others have persistent mild elevations.
A typical pattern is ALT higher than AST early on. If fibrosis or cirrhosis develops, AST may become equal to or higher than ALT. That shift is one reason persistent enzyme elevations should not be dismissed only because they are mild.
Alcohol-related liver injury
Alcohol can raise ALT, AST, and GGT. The classic pattern is AST higher than ALT, often with AST less than 300 U/L and an AST/ALT ratio above 2 in alcohol-associated hepatitis. GGT and enlarged red blood cells on a complete blood count may support the pattern, but no single lab proves alcohol is the cause.
The same amount of alcohol does not affect everyone equally. Risk rises with higher intake, binge patterns, obesity, viral hepatitis, certain medications, and existing liver disease. Alcohol and metabolic fatty liver can also overlap.
Medications and supplements
Many prescription drugs, over-the-counter medicines, and supplements can raise liver enzymes. Common examples include acetaminophen, certain antibiotics, anti-seizure medicines, statins, tuberculosis medicines, some antifungal medicines, anabolic steroids, and cancer therapies. Herbal and “natural” products can also cause liver injury.
Acetaminophen deserves special mention because overdose can cause severe, life-threatening liver injury. Risk is higher when someone exceeds the recommended dose, combines multiple acetaminophen-containing products, drinks heavy alcohol, or has poor nutrition. Suspected overdose needs urgent medical care even before symptoms appear.
Supplements are often missed because people may not think of them as medications. Green tea extract, turmeric or curcumin products, ashwagandha, black cohosh, garcinia cambogia, red yeast rice, bodybuilding products, and multi-ingredient weight-loss products have all been linked to liver injury in case reports or safety surveillance. The risk is unpredictable because dose, purity, labeling, and individual susceptibility vary.
Viral hepatitis and other infections
Hepatitis A, B, C, D, and E can raise ALT and AST. Acute hepatitis often causes larger elevations and may come with fatigue, nausea, dark urine, pale stools, jaundice, fever, or right upper abdominal discomfort. Chronic hepatitis B or C may cause mild, fluctuating, or even normal enzymes for long periods.
Other infections can also affect liver enzymes, including Epstein-Barr virus, cytomegalovirus, COVID-19, and sepsis. In these cases, the liver enzyme pattern must be interpreted with the overall illness.
Autoimmune, genetic, and metabolic liver diseases
Autoimmune hepatitis can cause mild, moderate, or marked enzyme elevations and may occur with high globulin or IgG levels. Hemochromatosis can raise ALT and AST along with high ferritin and transferrin saturation. Wilson disease, alpha-1 antitrypsin deficiency, and celiac disease are less common but important in selected people.
When ferritin is high with ALT or AST, the cause is not always iron overload. Ferritin can rise with inflammation, alcohol use, fatty liver, infection, and metabolic syndrome. A focused look at high ferritin and liver enzymes can help separate iron overload from inflammatory and metabolic patterns.
Muscle injury and intense exercise
Hard exercise can raise AST and ALT, especially after heavy resistance training, long endurance events, muscle trauma, or rhabdomyolysis. AST often rises more than ALT, and creatine kinase may be very high. This is one reason a liver workup can go off track if recent exercise is not mentioned.
If muscle injury is suspected, the pattern of CK, AST, and LDH may be more informative than repeating liver enzymes alone.
Other Tests That Change the Meaning
ALT and AST are most useful when interpreted with the rest of the panel. The same ALT can mean different things depending on bilirubin, ALP, GGT, albumin, INR, platelets, and symptoms.

Bilirubin deserves special attention. High ALT and AST with jaundice is more concerning than the same enzyme level with normal bilirubin. A bilirubin and liver enzyme pattern can help distinguish liver-cell injury from bile flow problems and red blood cell breakdown.
Albumin and INR also shift the level of concern. ALT and AST may show injury, but albumin and INR reflect liver synthetic function. A rising INR in acute hepatitis can be urgent because it may signal that the liver is struggling to make clotting factors.
For chronic mild elevations, fibrosis risk matters more than the enzyme number alone. A person with ALT of 65 U/L and low fibrosis risk is different from a person with ALT of 65 U/L, low platelets, diabetes, and abnormal liver stiffness. Noninvasive scores such as FIB-4 and APRI use routine lab values to estimate the chance of significant scarring, though they do not replace medical judgment.
When to Repeat, Test Further, or Seek Care
The next step depends on severity, symptoms, and context. Many mild elevations can be repeated after a short interval, especially if there is a clear temporary trigger. More significant or persistent abnormalities need a structured evaluation.
A repeat test is often reasonable when ALT or AST is mildly elevated, bilirubin and INR are normal, there are no concerning symptoms, and a likely explanation exists. Examples include recent intense exercise, a viral illness, alcohol intake, a new medication, or a short-term supplement. A clinician may suggest avoiding alcohol, holding nonessential supplements, reviewing medications, and retesting in a few weeks.
Further testing is usually considered when elevations persist, are more than mild, or occur with risk factors. Depending on the situation, this may include:
- Hepatitis A, B, C, and sometimes E testing
- Iron studies, including ferritin and transferrin saturation
- Autoimmune markers such as ANA, smooth muscle antibody, and IgG
- Metabolic tests such as fasting glucose, A1c, insulin resistance markers, and lipids
- Creatine kinase if muscle injury is possible
- Ultrasound or elastography to assess fat, bile ducts, and fibrosis risk
- Medication and supplement review, including exact doses and start dates
Prompt medical care is important if ALT or AST is high with jaundice, confusion, fainting, vomiting blood, severe abdominal pain, fever with worsening illness, easy bleeding, marked weakness, dark urine with pale stools, or a known acetaminophen overdose. Pregnancy with abnormal liver enzymes also needs timely evaluation because pregnancy-specific liver and blood pressure disorders can become serious.
Do not wait for symptoms if the numbers are very high. Acute liver injury can progress before a person feels severely ill. A clinician may need to check bilirubin, INR, kidney function, acetaminophen level, viral hepatitis tests, and other markers quickly.
How to Lower ALT and AST Without Overdoing It
ALT and AST usually improve when the underlying cause improves. The aim is not to “detox” the liver but to remove stressors, treat disease, and reduce long-term risk.
For metabolic fatty liver, the most reliable steps are gradual weight loss if needed, regular physical activity, better insulin sensitivity, and improved triglycerides. Even a 5% body weight reduction can lower liver fat in many people, while larger weight loss may be needed to improve inflammation or fibrosis. The best diet is one a person can sustain: often a Mediterranean-style pattern with vegetables, legumes, whole grains, fish, olive oil, nuts, and fewer refined carbohydrates and sugary drinks.
Exercise helps even without major weight loss. A practical target is at least 150 minutes per week of moderate activity plus two days of resistance training, adjusted for fitness and medical limits. Since intense exercise can temporarily raise AST and ALT, it helps to avoid unusually hard workouts for several days before repeat testing if the goal is to measure baseline liver enzymes.
For alcohol-related elevations, reducing or stopping alcohol can improve enzymes over weeks, but the safest plan depends on the level of intake. People who drink heavily every day should not suddenly stop without medical guidance because withdrawal can be dangerous. When liver disease is already present, abstinence is often the safest recommendation.
For medication or supplement-related elevations, the answer is not to stop every prescribed medicine on your own. Some drugs are essential, and stopping them can be riskier than the enzyme change. The safer approach is to review all prescriptions, over-the-counter medicines, herbals, powders, teas, and bodybuilding products with a clinician. Bring the bottles or take clear photos of the labels.
Be cautious with products marketed as liver cleanses. Many contain multi-ingredient herbal blends, laxatives, or concentrated extracts. They may not lower ALT or AST and can sometimes worsen liver injury. The liver does not need a cleanse to process normal waste products. It needs fewer toxins, better metabolic health, vaccination against hepatitis when appropriate, and treatment for specific diseases.
Sleep, thyroid disease, celiac disease, diabetes control, and medication interactions can also affect liver enzymes indirectly. A narrow focus on supplements can miss these bigger drivers.
Common Misreads and Real-World Examples
ALT and AST are easy to overread and underread at the same time. The number is important, but the setting gives it meaning.
One common mistake is assuming “normal” means perfect. A person with obesity, type 2 diabetes, low platelets, and ultrasound-confirmed fatty liver may still have normal ALT. Normal enzymes do not rule out fibrosis or cirrhosis.
Another mistake is assuming mild elevation means mild disease. A chronically elevated ALT of 60 U/L may look unimpressive, but if it has been present for years with diabetes and low platelets, fibrosis assessment matters.
A third mistake is blaming the liver when the signal is from muscle. Someone who performs a heavy leg workout, then tests the next day, may show AST higher than ALT. If CK is also high and bilirubin, ALP, and GGT are normal, muscle injury becomes more likely.
A fourth mistake is using the AST/ALT ratio as a diagnosis. An AST/ALT ratio above 2 can support alcohol-related injury in the right context, but it can also occur with cirrhosis or muscle injury. A ratio below 1 can occur in fatty liver and viral hepatitis, but it does not prove either one.
Consider a few simplified examples:
- ALT 68 U/L, AST 42 U/L, normal bilirubin, normal ALP, high triglycerides, A1c 6.1%. This pattern fits possible metabolic fatty liver, but viral hepatitis, alcohol, medications, and other causes still need consideration if persistent.
- AST 160 U/L, ALT 75 U/L, normal bilirubin, very high CK after a marathon. Muscle release may explain much of the pattern.
- ALT 920 U/L, AST 780 U/L, dark urine, nausea, high bilirubin. This needs prompt evaluation for acute hepatitis, drug-induced injury, toxin exposure, or another serious liver insult.
- AST 85 U/L, ALT 38 U/L, low platelets, high bilirubin, low albumin. The modest enzyme levels should not reassure anyone; the broader pattern may suggest advanced liver disease.
- ALT 110 U/L after starting a new supplement six weeks ago. The timing matters. Stopping nonessential supplements and reviewing ingredients with a clinician may be part of the workup.
The most useful interpretation combines five questions: How high are the numbers? Which enzyme is higher? Are bilirubin, ALP, GGT, albumin, INR, and platelets normal? Is the pattern new or persistent? What changed in the weeks to months before the test?
That approach keeps ALT and AST in their proper role: valuable signals, not standalone diagnoses.
