What ALT and AST Measure
ALT and AST are enzymes inside cells. Enzymes help the body run chemical reactions. ALT stands for alanine aminotransferase, and AST stands for aspartate aminotransferase. Older lab reports may call ALT “SGPT” and AST “SGOT.”
When liver cells become irritated, inflamed, injured, or leaky, ALT and AST can move from inside the cells into the bloodstream. A blood test then detects higher enzyme levels. This is why ALT and AST are often called “liver enzymes,” although only ALT is relatively liver-focused.
ALT is found mainly in liver cells, with smaller amounts in other tissues. Because of that, a high ALT often points toward liver cell injury, also called a hepatocellular pattern. AST is found in the liver too, but it is less specific because it is also present in skeletal muscle, heart muscle, kidneys, brain, pancreas, and red blood cells.
This difference is the reason an isolated AST result can be harder to interpret. A person who did heavy weight training, had a muscle injury, developed muscle inflammation, or had significant muscle breakdown may have high AST even when the liver is not the main problem. In that situation, a creatine kinase test, often called CK, can help separate muscle injury from liver injury. Patterns involving CK, AST, and LDH are especially useful when AST is higher than expected or symptoms include muscle pain, weakness, or dark urine.
ALT and AST are markers of liver cell injury, not direct measurements of liver performance. A liver can release enzymes while still making proteins, processing bilirubin, and supporting clotting normally. The reverse can also happen: advanced liver scarring may exist with only mild enzyme elevations or even normal ALT and AST. For liver function, doctors look more closely at albumin, INR, bilirubin, and clinical findings.

ALT usually deserves close attention when it stays above the reference range because it often reflects liver cell stress. A mild ALT increase can appear with metabolic dysfunction-associated steatotic liver disease, still commonly called fatty liver, especially when triglycerides, fasting glucose, insulin resistance, waist size, or body weight are also elevated.
AST deserves a wider lens. If AST is high but ALT is normal or only slightly elevated, the result does not automatically mean the liver is the source. Recent strenuous exercise, muscle injury, seizures, intramuscular injections, muscle disease, hemolysis, and some heart conditions can raise AST. In modern medicine, AST is no longer the main test for heart attack because cardiac troponins are more specific, but AST can still rise when muscle or other tissues are injured.
Reference ranges vary by lab, age, sex, body size, and testing method. Many laboratories list ALT and AST upper limits somewhere around 35–55 U/L, but some professional guidance uses lower “healthy normal” ALT cutoffs, especially for women. The safest way to read a result is to compare it with the reference interval printed on the same lab report and then look at the full pattern rather than one number. How to Read High ALT and High AST
The first step is to look at the size of the elevation. A result that is just above the upper limit of normal has a different meaning from a result that is 10, 20, or 50 times the upper limit.
Mild elevations are common. Many people discover ALT or AST elevations during routine blood work and feel completely well. Mild results can come from fatty liver, alcohol use, medications, supplements, recent exercise, viral infections, thyroid disease, celiac disease, or lab-to-lab variation. Doctors often repeat the test, review exposures, and check related markers before moving to advanced testing.
Moderate elevations raise the need for a more active search. Persistent or rising ALT and AST may lead to tests for hepatitis B and C, iron overload, autoimmune liver disease, metabolic risk, alcohol-associated injury, medication toxicity, and ultrasound findings. The exact workup depends on the pattern and the person’s history.
Very high elevations suggest more severe liver cell injury, especially when ALT and AST rise into the hundreds or thousands. Causes can include acute viral hepatitis, acetaminophen toxicity, severe drug-induced liver injury, ischemic hepatitis from poor blood flow to the liver, autoimmune hepatitis, and some bile duct problems that temporarily injure liver cells. Very high values should not be watched casually, especially if bilirubin or INR is abnormal.
The trend also matters. A single mildly high ALT may be less concerning than an ALT that keeps climbing over several weeks. A falling ALT after a known short-term trigger may be reassuring, but falling enzymes are not always good news if the liver’s function markers are worsening. In severe liver failure, enzymes can fall because fewer functioning liver cells remain to release them. That is why doctors compare ALT and AST with symptoms, bilirubin, INR, albumin, glucose, mental status, and kidney function.
The pattern is often more useful than the isolated number. ALT higher than AST suggests one group of causes. AST higher than ALT suggests another group. A mixed pattern with high alkaline phosphatase or bilirubin points in a different direction, often involving bile flow or obstruction rather than only liver cell leakage.
AST/ALT Ratio Patterns
The AST/ALT ratio compares AST with ALT. It is calculated by dividing AST by ALT. If AST is 80 U/L and ALT is 40 U/L, the ratio is 2.0. If AST is 35 U/L and ALT is 70 U/L, the ratio is 0.5.
The ratio can help, but it should never be used alone. It becomes most useful when both enzymes are abnormal and the clinical context fits the pattern.

In many liver conditions, ALT rises more than AST because ALT is more concentrated in the liver cell cytoplasm. This is common in metabolic fatty liver disease and viral hepatitis. In alcohol-associated liver injury, AST often rises more than ALT. One reason is that alcohol-related injury can involve mitochondria, where one form of AST is located. Alcohol use can also be associated with vitamin B6 deficiency, which may affect ALT activity more than AST activity.
An AST/ALT ratio above 2 is often discussed as a clue for alcohol-associated hepatitis, especially when GGT and bilirubin are also elevated and the person has a history of heavy alcohol use. Even then, the ratio does not prove the cause. Doctors still consider viral hepatitis, drug injury, biliary disease, cirrhosis, and muscle sources.
An AST/ALT ratio above 1 can also appear as liver fibrosis advances. In earlier fatty liver, ALT is often higher. As scarring progresses and liver cell mass changes, AST may catch up or become higher. This is one reason AST appears in fibrosis scores such as FIB-4 and APRI. These scores do not diagnose a specific liver disease, but they can help estimate whether advanced scarring is more or less likely. A broader comparison of FIB-4 and APRI can be helpful when liver enzymes remain abnormal or platelet count is low.
Common Causes of Abnormal ALT and AST
Many conditions can raise ALT and AST. The most likely cause depends on the level, ratio, symptoms, risk factors, and other blood tests.
Fatty liver and metabolic risk
Metabolic dysfunction-associated steatotic liver disease is one of the most common causes of mild to moderate ALT and AST elevation. It is linked with insulin resistance, type 2 diabetes, higher waist circumference, high triglycerides, low HDL cholesterol, high blood pressure, and excess liver fat. ALT is often higher than AST early on, but AST can become higher when fibrosis advances.
A person may have fatty liver even with only small enzyme changes. Some people with significant liver fat or fibrosis have normal ALT and AST. This is why liver enzymes are only one part of the picture. Metabolic markers, platelet count, fibrosis scores, and imaging may add important context.
Alcohol-associated liver injury
Alcohol can raise liver enzymes in several ways, including fatty change, inflammation, alcoholic hepatitis, and cirrhosis. AST is often higher than ALT in alcohol-associated patterns, and GGT may also be elevated. Alcohol-associated hepatitis often has jaundice and systemic illness, not just an abnormal lab result.
The amount and pattern of alcohol intake matter. Binge drinking, daily heavy use, and long-term intake all affect risk. So do sex, body weight, genetics, nutrition, viral hepatitis, and metabolic risk. When alcohol contributes to abnormal results, reducing or stopping alcohol can improve enzymes, but people with heavy daily use may need medical help to avoid dangerous withdrawal.
Medications, supplements, and toxins
Many prescription medications, over-the-counter drugs, herbal products, and supplements can affect ALT and AST. Acetaminophen overdose is a classic cause of severe liver injury and can produce very high aminotransferases. Some antibiotics, anti-seizure medicines, tuberculosis medications, cancer therapies, anabolic steroids, and immune-related drugs can also cause liver injury.
Supplements are often overlooked. Green tea extract, bodybuilding supplements, high-dose vitamin A, kava, black cohosh, ashwagandha, turmeric extracts, and multi-ingredient weight-loss products have all been linked to liver injury in some reports. “Natural” does not mean liver-safe. Patterns involving acetaminophen levels and liver enzymes need urgent medical interpretation when overdose is possible.
Viral hepatitis and infections
Hepatitis A, B, C, and E can raise ALT and AST. Acute viral hepatitis may cause high enzyme levels along with fatigue, nausea, abdominal discomfort, dark urine, pale stool, and jaundice. Chronic hepatitis B or C may cause fluctuating mild or moderate elevations, and some people have few symptoms.
Other infections can also affect liver enzymes, including Epstein-Barr virus, cytomegalovirus, and some systemic illnesses. Travel, food exposure, sexual exposure, injection drug use, transfusion history, tattoos, occupational exposure, and household contacts can shape the testing plan.
Muscle injury and intense exercise
AST can rise from skeletal muscle. ALT can rise too, but AST often rises more. Heavy lifting, endurance events, crush injury, seizures, muscle inflammation, statin-associated muscle injury, and rhabdomyolysis can all raise AST. Muscle-related patterns may include high CK, muscle pain, weakness, swelling, or dark urine.
This matters because a person with high AST after a hard workout may not need the same liver workup as someone with high AST, high bilirubin, low platelets, and jaundice. Timing helps. If blood was drawn within days of unusually intense exercise, a clinician may repeat liver enzymes after rest and check CK if needed.
Autoimmune, genetic, and less common liver diseases
Autoimmune hepatitis, hemochromatosis, Wilson disease, alpha-1 antitrypsin deficiency, celiac disease, thyroid disease, and bile duct disorders can cause abnormal liver enzymes. These are not the most common causes for every person, but they become more important when elevations persist, are unexplained, occur at a young age, run in families, or appear with other abnormal markers.
Iron studies can help screen for iron overload. Autoimmune markers may be considered when ALT and AST are persistently high. Ceruloplasmin and copper testing may be considered in younger people when Wilson disease is a concern. The workup should match the person, not just the enzyme number.
Other Tests That Change the Meaning
ALT and AST become far more useful when read with the rest of the liver panel. A hepatic function panel commonly includes ALT, AST, alkaline phosphatase, bilirubin, albumin, and sometimes total protein. A broader liver function tests panel may include GGT and clotting markers depending on the situation.
Alkaline phosphatase, or ALP, points more toward bile ducts or bone than liver cell leakage. If ALP is high along with bilirubin, doctors think about cholestasis, bile duct obstruction, gallstones, bile duct inflammation, infiltrative liver disease, or certain medication effects. GGT helps confirm whether a high ALP is more likely from liver or bile ducts rather than bone.
Bilirubin adds urgency and direction. High bilirubin can cause jaundice, dark urine, and pale stool. When bilirubin rises with ALT and AST, doctors consider hepatitis, drug injury, bile duct obstruction, and more serious liver stress. Patterns involving bilirubin and liver enzymes often need more prompt evaluation than a mild isolated ALT elevation.
Albumin and INR reflect liver function more directly than ALT and AST. Albumin is a protein made by the liver, although it can also be low from kidney loss, inflammation, malnutrition, or other illness. INR reflects clotting tendency and can rise when the liver cannot make clotting factors properly, when vitamin K is low, or when someone takes warfarin. Abnormal albumin and INR can be more concerning than the enzyme level alone.
Platelet count also matters. Low platelets can occur for many reasons, but in chronic liver disease they may suggest portal hypertension or advanced fibrosis. This is why platelet count is included in common fibrosis scores.
GGT can add useful context when AST is higher than ALT or alcohol-related injury is being considered. A high GGT is not specific for alcohol; it can rise with fatty liver, bile duct disease, medications, and other liver stress. Still, the pattern of GGT and ALT can make the interpretation more practical.
What to Do After Abnormal Results
Abnormal ALT or AST results should be handled calmly but not ignored. The right next step depends on how high the result is, whether it is new, whether symptoms are present, and whether other markers are abnormal.
For a mild, unexpected elevation, many clinicians repeat the test after a short interval. Before repeating, they may ask about alcohol, recent intense exercise, new medications, supplements, acetaminophen use, viral symptoms, travel, metabolic risk, and family history. Avoiding heavy exercise for several days before repeat testing can help if muscle contribution is possible. Stopping alcohol temporarily may clarify whether alcohol is contributing, but people with dependence should seek medical guidance before abrupt cessation.
A medication and supplement review is often one of the highest-value steps. Bring the actual bottles or a complete list, including dose and start date. Include over-the-counter pain relievers, herbal products, powders, teas, injections, bodybuilding products, and weight-loss supplements. Liver injury can appear weeks or months after a product is started, and multi-ingredient supplements can make the cause difficult to identify.
If enzymes stay elevated, the next layer may include hepatitis B and C testing, iron studies, fasting glucose or A1c, lipid testing, autoimmune markers, ultrasound, CK, thyroid testing, celiac testing, or fibrosis scoring. People with obesity, diabetes, prediabetes, high triglycerides, or metabolic syndrome may need assessment for fatty liver and fibrosis risk even when enzyme elevations are mild.
Lifestyle steps can improve many common ALT and AST patterns, especially fatty liver and alcohol-related changes. The most useful changes usually include reducing alcohol, losing weight if needed, improving insulin resistance, increasing physical activity, treating high triglycerides or diabetes, and avoiding unnecessary supplements. Even a 5% body weight reduction can improve liver fat in many people with fatty liver, while larger sustained losses may improve inflammation and scarring risk.
Do not try to “detox” the liver with cleanses. The liver does not need harsh cleanses, and some detox products can worsen liver injury. A safer plan is to remove avoidable stressors, identify the cause, treat metabolic risks, vaccinate against hepatitis A or B when appropriate, and follow results over time.
When to Seek Urgent Care
Some ALT and AST patterns need urgent medical attention, especially when symptoms suggest acute liver injury or liver failure. Seek urgent care promptly if abnormal liver enzymes occur with yellow skin or eyes, confusion, severe sleepiness, vomiting blood, black stools, severe right-upper-abdominal pain, fainting, severe weakness, high fever, new easy bruising or bleeding, or a swollen abdomen.
Possible acetaminophen overdose is an emergency, even if the person feels well at first. Liver injury can worsen after a delay, and early treatment works best. Urgent care is also needed after ingestion of poisonous mushrooms, toxic chemicals, or unknown pills, or when a medication reaction causes rash, fever, facial swelling, jaundice, or severe illness.
Very high ALT or AST should be interpreted quickly, especially when values are in the many hundreds or thousands. The exact number is not the only issue. Bilirubin, INR, kidney function, blood sugar, mental status, and the trend over hours to days can change the level of concern.
People with known cirrhosis should take new liver enzyme changes seriously, even if the numbers are not dramatic. Cirrhosis can reduce the liver’s reserve, so infection, bleeding, alcohol relapse, medication toxicity, dehydration, bile duct obstruction, or blood-flow problems may cause faster deterioration.
Pregnancy adds another layer. High liver enzymes with high blood pressure, severe headache, right-upper-abdominal pain, visual changes, swelling, low platelets, or feeling very unwell can suggest pregnancy-related liver or blood pressure complications and needs urgent obstetric evaluation.
For everyone else, persistent ALT or AST elevation deserves follow-up rather than guesswork. The most useful question is not “Which enzyme is worse?” but “What pattern do these enzymes form with the rest of the liver panel, symptoms, medications, alcohol exposure, metabolic health, and muscle markers?” That pattern usually points to the safest and most efficient next step.
