A valproic acid blood test measures the amount of valproate medicine in the bloodstream. Doctors use it most often for people taking valproic acid, divalproex sodium, or sodium valproate for seizures, bipolar disorder, or migraine prevention. The result helps show whether the dose is likely to control symptoms without causing side effects. Valproate is useful because it can calm overactive nerve signaling, but the safe and effective dose can vary widely from person to person.

This test is usually interpreted with timing, symptoms, dose history, liver tests, platelet count, albumin, pregnancy status when relevant, and other medicines. A number alone does not prove that a dose is right or wrong. A result that looks “normal” can still be unsafe in someone with confusion, severe drowsiness, low albumin, or liver disease. A high result can be urgent, especially after an overdose or when neurologic symptoms appear.

  • The usual total valproic acid therapeutic range is about 50–100 mcg/mL for seizure treatment, and some labs use up to 125 mcg/mL for mania.

What the Valproic Acid Blood Test Measures

The valproic acid blood test measures the concentration of valproate in blood. Valproate is the active form found after taking medicines such as valproic acid, divalproex sodium, sodium valproate, and several brand-name versions. The test is part of therapeutic drug monitoring, which means the blood level helps guide safe dosing for a medicine with a narrow usable range.

Valproate levels matter because the same dose can produce different blood concentrations in different people. Body weight, age, liver function, albumin level, other medicines, pregnancy, dose timing, formulation, missed doses, and illness can all change the result. Two people taking the same milligram dose may have very different blood levels.

The test is commonly ordered when:

  • Starting valproate or changing the dose
  • Checking whether a level is in the therapeutic range
  • Investigating breakthrough seizures or mood symptoms
  • Looking for toxicity when side effects appear
  • Checking adherence when missed doses are possible
  • Reviewing a drug interaction
  • Monitoring someone with liver disease, kidney disease, low albumin, or critical illness
  • Following a suspected overdose

Valproic acid testing is usually not interpreted alone. It is often paired with dose history, seizure or mood control, side effects, and safety labs. In seizure care, it may be reviewed alongside other seizure medication levels if a person takes more than one antiseizure drug.

The result is usually reported in mcg/mL, which is the same numeric value as mg/L. For example, 80 mcg/mL equals 80 mg/L. Some laboratories also report values in micromoles per liter, but mcg/mL is common in patient-facing results.

Therapeutic Range, Toxic Level, and Result Patterns

The total valproic acid therapeutic range is usually 50–100 mcg/mL for seizure treatment. Some laboratories and psychiatric uses, especially mania treatment, use a wider range such as 50–125 mcg/mL. A result inside the range means the blood level is in the expected treatment zone, not that the dose is automatically perfect.

A “toxic level” is less exact. Side effects can happen within the therapeutic range, especially in sensitive people, older adults, people with low albumin, or those taking interacting medicines. Toxicity becomes more likely as the level rises above the upper range. Levels above about 150 mcg/mL are generally concerning, and levels around 175 mcg/mL or higher are often treated as potentially toxic, especially if symptoms are present.

The most useful interpretation asks three questions: Was the blood drawn at the right time? Is the patient clinically well? Are safety labs reassuring? A level of 105 mcg/mL drawn soon after a dose may not mean the same thing as a true trough of 105 mcg/mL. A level of 85 mcg/mL may still be too high for someone with severe sleepiness and a high free valproate level.

Valproate also behaves differently from some other seizure medicines because it is highly protein bound. Only the unbound, or “free,” portion is pharmacologically active. When albumin is low, the total level may look acceptable while the active free level is too high.

When and How the Test Is Done

A valproic acid blood test is a standard blood draw from a vein. Fasting is usually not required. The most important preparation is accurate timing and a clear dose history.

For routine monitoring, the preferred sample is usually a trough level, drawn shortly before the next scheduled dose. Trough testing gives a steadier comparison from one result to the next. If a person takes valproate twice daily, the sample is often drawn before the morning dose. If the person takes an extended-release form once daily, the timing may differ based on the prescriber’s instructions and the lab’s protocol.

Tell the clinician or lab:

  • The exact valproate product and dose
  • The time of the last dose
  • Whether any doses were missed
  • Whether vomiting or diarrhea occurred
  • Any new medicines, antibiotics, supplements, or alcohol use
  • Whether the test is a routine trough, urgent toxicity check, or overdose evaluation

Levels are often checked after the medicine reaches steady state, commonly several days after starting or changing a dose. In stable long-term treatment, levels may be checked only periodically or when something changes. The schedule can become more frequent during pregnancy, medication changes, new liver problems, suspected toxicity, or poor symptom control.

Valproate results are sometimes compared with peak and trough levels for other medicines, but valproate routine monitoring usually focuses on trough concentration rather than peak concentration. A random level may still be useful in emergency care, especially after overdose or when serious symptoms appear, but it is harder to compare with a therapeutic range.

Do not stop or change valproate before the test unless the prescribing clinician gives that instruction. Suddenly stopping an antiseizure medicine can trigger seizures, including severe or prolonged seizures.

High Valproic Acid Levels

A high valproic acid level means the measured concentration is above the lab’s expected range or higher than the person’s target. The most common reasons are dose-related: the dose is too high, the blood was drawn too soon after a dose, or the body is clearing the medicine more slowly than expected.

Common causes of a high result include:

  • Recent dose increase
  • Blood draw soon after taking the medicine
  • Liver disease or reduced liver metabolism
  • Older age or frailty
  • Low albumin, which can raise the free active fraction
  • Drug interactions that increase valproate exposure
  • Acute illness, dehydration, or poor oral intake
  • Accidental double dosing
  • Intentional or accidental overdose

High levels can cause mild, moderate, or life-threatening symptoms. Early symptoms may include tremor, dizziness, nausea, vomiting, unsteady walking, blurred vision, or unusual tiredness. More serious toxicity may cause confusion, marked sleepiness, agitation, low body temperature, breathing problems, low blood pressure, metabolic acidosis, high ammonia, seizures, coma, or death.

A high total level also raises concern for platelet suppression and bleeding risk. Valproate can lower platelet count, and this risk tends to increase at higher concentrations. A complete blood count helps check platelets, red blood cells, and white blood cells during treatment.

Urgent care is appropriate when a high level occurs with severe symptoms. Do not wait for a repeat outpatient test if the person is difficult to wake, confused, breathing slowly, repeatedly vomiting, having seizures, showing jaundice, or has taken a possible overdose. Emergency treatment may include observation, repeat levels, electrolytes, acid-base testing, ammonia testing, activated charcoal in selected recent ingestions, L-carnitine in selected severe cases, and dialysis in severe poisoning.

Low Valproic Acid Levels

A low valproic acid level means the concentration is below the expected treatment range. In seizure treatment, a level below about 50 mcg/mL may increase the risk of breakthrough seizures, but some people remain controlled at lower concentrations. The number must be interpreted with the person’s history.

Common reasons for a low level include:

  • Missed or late doses
  • Taking the blood sample long after the last dose
  • Recent dose reduction
  • Taking less medicine than prescribed because of side effects
  • Vomiting soon after a dose
  • Trouble absorbing medicine
  • Switching between formulations without dose adjustment
  • Drug interactions that lower valproate levels
  • Rapid clearance in some individuals

One major interaction deserves special attention: carbapenem antibiotics can sharply lower valproate concentrations and may make seizure control worse. Examples include meropenem, imipenem, ertapenem, and doripenem. This interaction can be strong enough that simply increasing the valproate dose may not fix the problem while the antibiotic is being used.

Other antiseizure medicines can also affect valproate levels, and valproate can affect them in return. For example, phenytoin and carbamazepine have their own therapeutic ranges, side effect patterns, and interaction issues. If someone takes multiple antiseizure medicines, clinicians may compare valproate with a phenytoin blood test or carbamazepine blood test when adjusting therapy.

A low result should not automatically lead to a higher dose. First, confirm timing and adherence. A level drawn at the wrong time can look low even when the usual trough is acceptable. The clinician may repeat the test as a properly timed trough before changing the dose, especially if symptoms are controlled.

Monitoring Safety: Liver, Platelets, Ammonia, and Pregnancy

Valproic acid monitoring is not only about the drug level. Safety monitoring is just as important because serious side effects can occur even when the blood concentration is not dramatically high.

Liver safety is one of the main concerns. Valproate can cause liver enzyme elevations and, rarely, severe liver injury. Risk is higher in young children, people taking multiple antiseizure medicines, and people with certain mitochondrial disorders. Clinicians often use liver function tests before and during treatment, especially early in therapy or when symptoms appear.

Symptoms that may suggest liver or pancreas problems include:

  • Unusual weakness or extreme fatigue
  • Loss of appetite
  • Persistent nausea or vomiting
  • Right upper abdominal pain
  • Yellow skin or eyes
  • Dark urine
  • Severe abdominal pain that spreads to the back

Platelet monitoring is also important. Valproate can cause thrombocytopenia, which means a low platelet count. Platelets help blood clot. Low platelets can cause easy bruising, nosebleeds, bleeding gums, heavy menstrual bleeding, or prolonged bleeding from cuts. Higher valproate levels, older age, and female sex may increase this risk.

Ammonia is another safety issue. Valproate can cause high ammonia, sometimes with normal liver enzymes. High ammonia can lead to encephalopathy, a brain-function problem that may cause confusion, sleepiness, vomiting, behavior change, or worsening seizures. A blood ammonia test is not always ordered routinely, but it becomes important when mental status changes occur.

Pregnancy and reproductive safety need careful planning. Valproate can cause major birth defects and neurodevelopmental harm when used during pregnancy. People who can become pregnant should have clear counseling about pregnancy prevention, alternatives, and what to do before trying to conceive. Do not stop valproate suddenly because uncontrolled seizures can also be dangerous. The safest plan is made before pregnancy whenever possible.

Free vs Total Valproic Acid

Most routine tests measure total valproic acid. Total valproic acid includes both protein-bound medicine and free medicine. The free portion is the active portion that can enter tissues and cause both treatment effects and toxicity.

In many stable adults with normal albumin, total valproic acid is enough. But total levels can mislead when protein binding changes. Valproate is highly bound to albumin, the main blood protein that carries many medicines. When albumin is low, a larger percentage of valproate may be free. In that situation, the total level can look normal while the active free level is high.

Free valproic acid testing may be useful when:

  • Albumin is low
  • Kidney disease is present
  • Liver disease is present
  • The person is critically ill
  • Pregnancy changes drug handling
  • Symptoms suggest toxicity despite a normal total level
  • Total levels are high but symptoms are mild
  • Other highly protein-bound medicines are being used

A common free valproic acid reference range is roughly 5–25 mcg/mL, though some laboratories use slightly different limits, such as about 6–22 mcg/mL. The exact range should be read from the reporting lab because methods and cutoffs vary.

Free testing is especially helpful when the clinical picture and total level do not match. For example, a total valproate level of 75 mcg/mL may look therapeutic. If albumin is very low and the person is confused, a free level may reveal excessive active drug. In contrast, a total level slightly above range may be less concerning if the free level and symptoms are acceptable, though this decision belongs to the treating clinician.

What to Do With Your Results

A valproic acid result should be reviewed with the person who prescribes the medicine. The best next step depends on the number, timing, symptoms, reason for treatment, and safety labs.

For a result in range, the dose may stay the same if seizures or mood symptoms are controlled and side effects are absent. Continued monitoring may include periodic drug levels, liver tests, platelet counts, and symptom review.

For a low result, the clinician may ask about missed doses, refill gaps, vomiting, diarrhea, timing, or interacting medicines. A repeat trough may be ordered before a dose change. If seizures have returned, a faster clinical review is usually needed.

For a high result, the clinician may lower the dose, hold a dose, repeat a trough, check free valproate, or order safety labs. If symptoms are significant, the result should be treated as a medical problem, not just a lab abnormality.
