The Fibrosis-4 Index, usually called FIB-4, is a simple liver fibrosis score calculated from age, AST, ALT, and platelet count. It does not measure liver scarring directly. Instead, it uses routine blood test results to estimate whether advanced liver fibrosis is unlikely, possible, or more likely. That makes it useful as a first step when someone has fatty liver disease, type 2 diabetes, metabolic syndrome, hepatitis, abnormal liver enzymes, or another reason to check for liver scarring.

A low FIB-4 score is reassuring, but it does not prove the liver is perfectly healthy. A high score does not diagnose cirrhosis by itself, but it usually deserves follow-up with a clinician, repeat labs, elastography, or a liver specialist referral. The score is most helpful when it is interpreted with the person’s age, medical history, alcohol intake, medications, liver enzyme pattern, and platelet count trend.

What the FIB-4 Index Measures

FIB-4 is a noninvasive fibrosis score. “Noninvasive” means it uses information already available from blood tests instead of requiring a liver biopsy. “Fibrosis” means scar-like tissue that can build up in the liver after long-term inflammation or injury. Mild fibrosis may have little effect on daily life. Advanced fibrosis is more serious because it can progress toward cirrhosis, portal hypertension, liver failure, and liver cancer risk.

FIB-4 was first developed in people with hepatitis C and HIV coinfection, but it is now widely used in fatty liver disease and metabolic dysfunction-associated steatotic liver disease, often shortened to MASLD. MASLD is the newer name for what many people still know as nonalcoholic fatty liver disease, or NAFLD.

The score works because advanced liver scarring often changes routine blood test patterns. AST may rise, ALT may rise or fall depending on the stage and cause of liver injury, and platelet count may drop as scarring affects blood flow through the liver and spleen. Age is included because fibrosis risk rises over time in many chronic liver conditions.

FIB-4 does not measure fat in the liver. It also does not show whether liver inflammation is active today. A person can have a low FIB-4 and still have fatty liver. A person can have a high FIB-4 without having cirrhosis. The score is best understood as a sorting tool: it helps decide who is unlikely to have advanced fibrosis and who needs a closer look.

FIB-4 is often reviewed alongside liver enzymes, especially AST and ALT. If those markers are confusing, a plain-language review of ALT and AST liver enzyme patterns can help show why the same enzymes can mean different things in different people.

FIB-4 Formula and Calculator Inputs

FIB-4 uses four inputs:

  • Age in years
  • AST in U/L
  • ALT in U/L
  • Platelet count in 10⁹/L, which is numerically the same as 10³/µL in many lab reports

The formula is:

FIB-4 = (age × AST) ÷ (platelet count × sqrt(ALT))

The score has no unit. It is a number used for risk grouping.

For example, imagine a 52-year-old person with:

  • AST: 48 U/L
  • ALT: 60 U/L
  • Platelets: 210 × 10⁹/L

The calculation is:

FIB-4 = (52 × 48) ÷ (210 × sqrt(60))

That equals about 1.53. In many adult fatty liver pathways, that falls into the middle or indeterminate range, meaning it is not clearly low risk and not clearly high risk. It would usually be interpreted with the person’s risk factors and may lead to repeat testing or a second noninvasive fibrosis test.

Each part of the formula matters in a different way. Age raises the score even if the lab values stay the same. AST raises the score directly. Platelets lower the score when they are higher and raise the score when they are lower. ALT is in the denominator as a square root, so very high ALT can sometimes make FIB-4 look lower than expected during acute liver injury.

That last detail is easy to miss. A person with active hepatitis or a sudden medication-related liver injury can have very abnormal enzymes but a FIB-4 score that does not reflect stable long-term fibrosis risk. FIB-4 was not designed to replace clinical judgment during acute illness.

Platelets deserve special attention because they are part of many liver fibrosis scores. A falling platelet count can occur for several reasons, including liver scarring, enlarged spleen, immune conditions, viral infections, medications, alcohol use, bone marrow disorders, and lab variation. The meaning of the platelet part of FIB-4 is easier to judge when compared with the person’s full blood count and platelet trend. A separate review of the platelet count blood test normal range can help put this part of the score into context.

FIB-4 Normal Range, Low Risk, and High Risk Cutoffs

FIB-4 does not have a “normal range” in the same way that sodium, hemoglobin, or albumin does. It is a calculated risk score. The result is usually grouped into low, intermediate, or high risk for advanced fibrosis.

Many adult MASLD and NAFLD pathways use these common cutoffs:

These cutoffs are not universal for every disease, every age group, or every clinical setting. Older hepatitis C studies often used different thresholds, such as below 1.45 to help rule out advanced fibrosis and above 3.25 to help rule it in. Fatty liver pathways often use below 1.3 and above 2.67 because those thresholds work better for primary care triage in many adults with metabolic risk.

Age changes interpretation. In adults older than 65, a cutoff around 2.0 is often used as a more practical low-risk threshold because age alone can push the score upward. Without age adjustment, many older adults would be placed in the indeterminate range even when their fibrosis risk is not truly high.

FIB-4 is less reliable in adults younger than about 35. Younger people can have important liver disease with a deceptively low score because the age part of the formula pulls the result down. A young adult with type 2 diabetes, obesity, high liver enzymes, heavy alcohol use, viral hepatitis, or a strong family history of liver disease may still need further evaluation even if FIB-4 is low.

The “best” cutoff also depends on the purpose. A low cutoff is useful when the aim is to avoid missing advanced fibrosis. A high cutoff is useful when the aim is to identify people who deserve more urgent follow-up. The middle range exists because FIB-4 is not precise enough to classify everyone.

What Low, Borderline, and High FIB-4 Results Can Mean

A FIB-4 result should be read as a probability signal, not a final diagnosis. The same number can mean different things depending on why it was checked.

Low FIB-4

A low FIB-4 usually means advanced liver fibrosis is unlikely at that moment. This is especially helpful in primary care because many people with fatty liver or mildly elevated liver enzymes do not have advanced scarring. A low score can reduce unnecessary referrals and testing when the rest of the clinical picture is reassuring.

Low risk does not mean no risk. FIB-4 can miss disease in younger adults, in people with early fibrosis, and in some people whose platelets and enzymes have not changed much. A low result also does not explain why ALT or AST is high. If liver enzymes remain abnormal, the cause still matters.

A low score is usually interpreted with metabolic risk factors. Someone with type 2 diabetes, high triglycerides, high blood pressure, central weight gain, or known fatty liver may need periodic reassessment even if the first FIB-4 is low. In many care pathways, repeat testing every one to three years is considered, with shorter intervals for higher-risk metabolic profiles.

Intermediate FIB-4

An intermediate result is common. It means FIB-4 alone cannot confidently sort the person into low or high risk. This does not mean the liver is definitely scarred. It means the score has reached a gray zone.

In this range, clinicians often look at:

  • Whether AST and ALT have been stable or rising
  • Whether platelet count has been falling
  • Whether diabetes, obesity, or metabolic syndrome is present
  • Whether alcohol intake could be contributing
  • Whether viral hepatitis, iron overload, autoimmune liver disease, or medication injury should be considered
  • Whether elastography, an ELF test, FibroTest, or specialist review is available

A middle-range FIB-4 is one reason other fibrosis scores may be compared. For example, FIB-4 and APRI both use AST and platelet information, but they are not identical. APRI does not include age or ALT, so it can behave differently in the same person.

High FIB-4

A high FIB-4 raises concern for advanced fibrosis, but it does not prove it. It is a signal to check the result carefully and move to a more specific assessment.

A high score may occur because of true liver scarring. It may also occur because of a temporary AST rise, a low platelet count from another cause, recent heavy alcohol exposure, acute hepatitis, intense exercise with muscle injury, or lab timing during illness. The higher the score and the stronger the risk factors, the more seriously it should be taken.

High FIB-4 often leads to additional testing such as transient elastography, sometimes known by the brand name FibroScan, or magnetic resonance elastography when available. Clinicians may also check a broader liver panel, viral hepatitis tests, iron studies, autoimmune markers, and imaging depending on the pattern.

A high score with low albumin, high INR, high bilirubin, swelling, ascites, confusion, vomiting blood, black stools, or jaundice deserves prompt medical attention. Those features are not just fibrosis screening issues; they may suggest impaired liver function or complications of advanced liver disease. In that situation, FIB-4 should not delay urgent care.

When FIB-4 Is Most Useful

FIB-4 is most useful as a first-pass fibrosis risk check in adults with stable blood test results. It is commonly used in fatty liver disease because MASLD is common, many people have no symptoms, and advanced fibrosis is the subgroup that needs more active follow-up.

The score is often helpful when someone has:

  • Known fatty liver on ultrasound, CT, MRI, or prior records
  • Type 2 diabetes or prediabetes
  • Metabolic syndrome
  • Long-term elevated ALT or AST
  • Obesity or central weight gain
  • High triglycerides or low HDL cholesterol
  • Chronic hepatitis B or hepatitis C
  • A history of alcohol-related liver stress
  • A need for liver fibrosis triage before referral

In a busy clinic, FIB-4 helps separate people who can often be monitored from those who should move to second-line fibrosis testing. This is valuable because liver enzymes alone are poor fibrosis markers. Some people with advanced fibrosis have only mild enzyme abnormalities. Others have high enzymes from inflammation without advanced scarring.

FIB-4 is also useful because it is inexpensive. It uses routine results that are often already available from a complete blood count and a chemistry or liver panel. When AST, ALT, and platelets are already checked, the score adds no extra blood draw.

A broader liver function tests panel can show enzyme, bilirubin, and protein patterns that FIB-4 does not include. That matters because FIB-4 focuses on fibrosis risk, while a liver panel can point toward hepatocellular injury, bile duct obstruction, jaundice patterns, or reduced protein production.

FIB-4 is not the same as an AST/ALT ratio. The AST/ALT ratio compares two enzymes and may suggest certain patterns, such as alcohol-related liver injury or advanced liver disease in the right context. FIB-4 combines enzymes with age and platelets, making it more focused on fibrosis triage. When AST is higher than ALT, the broader AST/ALT ratio pattern may add useful context, but it still does not replace fibrosis assessment.

What to Do After a FIB-4 Result

The next step depends on the score, the reason it was checked, and whether the person is clinically stable.

For a low FIB-4, the usual next step is not to ignore liver health. It is to manage the underlying risk and repeat assessment when appropriate. In MASLD, that often means addressing weight, waist circumference, blood sugar, triglycerides, blood pressure, sleep apnea, alcohol intake, and medications that may affect the liver. Even modest weight loss can improve liver fat in many people, while larger sustained weight loss is more likely to improve inflammation and fibrosis risk.

For an intermediate FIB-4, the next step is often repeat labs or a second noninvasive fibrosis test. Repeat labs can be helpful when the score may have been distorted by a temporary illness, alcohol exposure, intense exercise, medication change, or lab error. If the result remains in the gray zone, elastography or another fibrosis marker can help clarify risk.

For a high FIB-4, follow-up should usually be more active. Many clinicians confirm the inputs, review the platelet count trend, check for acute causes of AST elevation, and order second-line fibrosis assessment. A referral to hepatology or gastroenterology is often appropriate, especially when the result is clearly high, persistent, or combined with abnormal imaging or signs of impaired liver function.

A practical follow-up conversation may include these questions:

  • Were AST, ALT, and platelets measured during a stable period?
  • Has platelet count been falling over time?
  • Is there known fatty liver, hepatitis, heavy alcohol exposure, or diabetes?
  • Are there symptoms or signs of cirrhosis, such as fluid buildup, easy bruising, jaundice, or confusion?
  • Is elastography available?
  • Are other causes of liver disease being considered?

FIB-4 should also be interpreted with liver synthetic function when advanced disease is a concern. Albumin and INR are not part of the FIB-4 formula, but they help show how well the liver is making proteins and clotting factors. A separate review of albumin and INR in liver synthetic function can help explain why these markers matter when fibrosis is more advanced.

If fatty liver appears alongside high ferritin, abnormal liver enzymes, or metabolic syndrome, clinicians may also check iron studies and inflammation patterns. High ferritin does not always mean iron overload; it can rise with fatty liver, alcohol use, inflammation, and metabolic disease. The combination of high ferritin and liver enzymes often needs a more careful pattern-based interpretation.

Common Mistakes and Limitations

The most common mistake is treating FIB-4 as a diagnosis. It is not a diagnosis of fatty liver, MASH, hepatitis, cirrhosis, or liver failure. It estimates fibrosis risk from a small set of variables.

Another common mistake is using FIB-4 during acute illness as if it reflects long-term liver scarring. AST and ALT can rise quickly during viral illness, medication injury, alcohol-related injury, gallbladder problems, ischemic injury, or strenuous exercise. Platelets can fall during infection, inflammation, immune reactions, or bone marrow stress. When those changes are temporary, FIB-4 can be misleading.

A third mistake is ignoring age. Because age is in the numerator, the same lab values produce a higher score in an older adult. This does not make the score useless, but it means older adults need age-aware interpretation. In younger adults, the opposite problem occurs: age can make the score look falsely reassuring.

FIB-4 is also less useful in children and teenagers because it was not designed for pediatric liver assessment. Pediatric fatty liver disease and inherited liver conditions need different evaluation strategies.

The score may be unreliable when platelet count is abnormal for non-liver reasons. A person with immune thrombocytopenia, chemotherapy-related low platelets, a bone marrow disorder, or certain medication effects may have a falsely high FIB-4. On the other hand, a person with early fibrosis but normal platelets may still have a low or middle-range score.

FIB-4 also does not describe the cause of liver injury. It cannot tell whether abnormal enzymes come from MASLD, alcohol, hepatitis B, hepatitis C, autoimmune hepatitis, hemochromatosis, Wilson disease, alpha-1 antitrypsin deficiency, medication injury, bile duct disease, celiac disease, thyroid disease, or muscle injury. The cause still needs to be found when the pattern calls for it.

The score is not a substitute for imaging when imaging is needed. Ultrasound, CT, MRI, transient elastography, and magnetic resonance elastography answer different questions. Some look for fat, masses, bile duct dilation, spleen enlargement, fluid, or liver stiffness. FIB-4 only uses blood-based inputs.

The safest way to use FIB-4 is as part of a sequence:

  1. Confirm the blood test inputs and clinical context.
  2. Sort the result into low, indeterminate, or higher risk.
  3. Repeat or confirm if the result may be temporary or distorted.
  4. Use second-line fibrosis testing when the result is indeterminate or high.
  5. Treat the underlying liver and metabolic risk factors rather than chasing the score alone.

A changing FIB-4 trend can be useful, but only when the same kind of stable conditions apply each time. A gradual rise over years may matter more than a single mild change. A sudden jump should prompt a careful look at AST, ALT, platelets, alcohol intake, medications, infections, and recent exertion before assuming fibrosis has suddenly worsened.

FIB-4 is valuable because it is simple, low-cost, and widely available. Its strength is ruling out advanced fibrosis in many stable adults. Its weakness is that it cannot explain the whole liver story by itself.
