
A microscopic polyangiitis blood test panel is designed to recognize a pattern of small-vessel inflammation and, above all, identify kidney or lung injury before it becomes irreversible. MPO-ANCA is the antibody most strongly associated with microscopic polyangiitis, but a positive result is not diagnostic by itself and a negative result does not completely exclude the disease. CRP and ESR can show systemic inflammation, yet neither measures kidney damage directly. The decisive kidney-risk information comes from serum creatinine, estimated glomerular filtration rate, urinalysis, urine microscopy, and quantitative urine protein. When the urine shows glomerular blood or casts, kidney function is falling, or pulmonary hemorrhage is possible, evaluation is urgent. Kidney biopsy often confirms pauci-immune necrotizing crescentic glomerulonephritis and provides prognostic information, but treatment should not be delayed in a rapidly deteriorating, clinically convincing case.
- MPO-ANCA supports microscopic polyangiitis but must match the organ pattern.
- Urine microscopy and creatinine are more direct measures of kidney danger than CRP or ESR.
- Kidney inflammation may cause no pain and only microscopic blood at first.

The Panel Organized by Clinical Question
Microscopic polyangiitis, or MPA, is a necrotizing small-vessel vasculitis. It commonly affects glomerular capillaries in the kidneys and pulmonary capillaries in the lungs, but it can also involve skin, peripheral nerves, joints, muscles, and the gastrointestinal tract. Unlike granulomatosis with polyangiitis, it usually does not produce granulomatous masses in the upper airway or lungs.
A useful panel is not simply a list of autoimmune antibodies. Each group of tests answers a different clinical question.
Is ANCA-associated vasculitis likely?
- MPO-ANCA antibody
- PR3-ANCA antibody
- ANCA by indirect immunofluorescence when the laboratory uses it as a screen or reflex test
Are the kidneys inflamed or losing function?
- Serum creatinine and eGFR
- Urinalysis for blood and protein
- Microscopic urine examination for dysmorphic red cells and red blood cell casts
- Urine protein-to-creatinine or albumin-to-creatinine ratio
- Electrolytes, especially potassium and bicarbonate
Is there systemic inflammation or blood loss?
- CRP and ESR
- Complete blood count with differential and platelets
- Albumin and liver tests
Could another disease explain the findings?
- Anti-glomerular basement membrane antibody
- Complement C3 and C4
- ANA and double-stranded DNA antibodies when lupus is plausible
- Infection testing, blood cultures, viral serology, or echocardiography according to the presentation
- Serum protein studies or cryoglobulins in selected patients
This structure prevents a common error: focusing on the antibody while overlooking organ injury. A patient with a modest MPO-ANCA result and rapidly rising creatinine may be in greater danger than a patient with a very high antibody concentration and stable organs.
The sample should ideally be drawn before immunosuppression, but urgent treatment takes priority when pulmonary hemorrhage or rapidly progressive glomerulonephritis is likely. The date of corticosteroid, rituximab, cyclophosphamide, plasma exchange, or other therapy should be documented because treatment can alter later findings.
How to Interpret MPO-ANCA
MPO stands for myeloperoxidase, an enzyme stored in neutrophil granules. MPO-ANCA antibodies target this enzyme and are commonly detected by antigen-specific immunoassay. They are strongly associated with MPA and renal-limited pauci-immune glomerulonephritis, although they can also occur in eosinophilic granulomatosis with polyangiitis, drug-induced vasculitis, and less often other conditions.
A positive MPO-ANCA test has its highest diagnostic value when the patient has compatible objective findings: active glomerular urine sediment, pulmonary capillaritis, mononeuritis multiplex, palpable purpura, or another small-vessel vasculitic manifestation. The same result is less convincing in a person tested for nonspecific fatigue or joint discomfort.
Laboratories report MPO antibodies in assay-specific units. There is no universal numeric cutoff that can be transferred from one platform to another. Results may be negative, borderline, low positive, or clearly positive based on the manufacturer and laboratory validation. The degree of positivity influences likelihood, but even a strong result is not absolute proof.
False or non-vasculitic positives can occur with:
- Certain medications, including hydralazine, propylthiouracil, and some other drug exposures
- Chronic infection, including infective endocarditis in rare cases
- Autoimmune liver disease and inflammatory bowel disease
- Other systemic autoimmune diseases
- Some malignancies or chronic inflammatory states
Drug-induced disease may show very high MPO-ANCA, antibodies to additional neutrophil antigens, ANA, or low complement, but the pattern varies. A complete medication and exposure history is essential.
MPO-ANCA is not synonymous with p-ANCA. Perinuclear ANCA is a staining pattern seen on ethanol-fixed neutrophils; MPO is a specific antigen target. Many MPO-positive samples show p-ANCA, but atypical p-ANCA can target other proteins and occurs in non-vasculitic diseases. Modern diagnostic strategies emphasize MPO- and PR3-specific immunoassays.
A negative MPO-ANCA result does not completely exclude MPA. Some patients are ANCA-negative, and some have PR3-ANCA. When organ findings strongly suggest pauci-immune vasculitis, biopsy and the full differential diagnosis remain important.
Testing algorithms differ. Some laboratories begin directly with high-quality MPO- and PR3-specific immunoassays; others add indirect immunofluorescence when the result is negative, weak, discordant, or requested as part of a reflex panel. A report should be read as a complete set rather than by selecting the most alarming line. For example, an atypical fluorescence pattern with negative MPO and PR3 has much less support for MPA than a clearly positive MPO assay in a patient with glomerulonephritis.
How the Panel Detects Kidney Risk
Kidney disease is common in MPA and may progress quickly. The characteristic lesion is pauci-immune necrotizing and crescentic glomerulonephritis. “Pauci-immune” means that routine immunofluorescence shows little or no immune-complex deposition, distinguishing it from lupus nephritis, IgA nephritis, and many complement-mediated diseases.
Early symptoms may be absent. The kidneys themselves often do not hurt. A patient may feel tired, nauseated, short of breath, or swollen only after filtration has already declined. This is why the urine and creatinine are central to the panel.
Urinalysis: Blood and protein together raise concern for glomerular injury. A dipstick positive for blood should be confirmed microscopically because muscle pigment or free hemoglobin can also trigger the pad.
Urine microscopy: Dysmorphic red cells, especially acanthocytes, and red blood cell casts point toward glomerular bleeding. Casts are highly concerning but can be missed if the sample is delayed or the sediment is not examined by an experienced laboratory.
Creatinine and eGFR: A rising creatinine or falling eGFR shows loss of filtration. The trend can be more informative than whether one value falls inside a population reference interval. An older adult with low muscle mass may have serious kidney dysfunction despite a creatinine that appears only mildly elevated.
Urine protein: MPA usually produces moderate proteinuria, but heavier loss can occur. Quantification with a protein-to-creatinine ratio helps establish baseline severity and follow response. Massive proteinuria should also prompt consideration of an overlapping glomerular lesion.
Electrolytes and acid-base status: High potassium, low bicarbonate, and fluid overload can signal advanced or rapidly worsening kidney failure and may require urgent dialysis support.
Long-term kidney risk depends on the eGFR at presentation, speed of treatment response, age and comorbidity, relapse history, and biopsy findings. The proportion of normal glomeruli and the degree of chronic scarring often carry more prognostic information than the MPO-ANCA titer.
Validated renal-risk tools combine baseline eGFR with biopsy measures such as the percentage of normal glomeruli and interstitial fibrosis or tubular atrophy. These tools estimate group-level risk of kidney failure; they do not predict an individual outcome with certainty. They are most useful for counseling and follow-up planning after adequate biopsy tissue has been reviewed by renal pathology.
A patient with suspected MPA and new urinary blood should not be reassured by a normal CRP. Likewise, a high CRP without urine abnormalities does not prove kidney vasculitis. The organ-specific tests determine the immediate risk.
CRP, ESR, and Blood-Count Findings
CRP and ESR often rise during active MPA, particularly with constitutional symptoms or multi-organ disease. They are useful for establishing an inflammatory baseline and following trends, but they are not specific for vasculitis.
The CRP test responds relatively quickly to IL-6-driven liver signaling. Infection, surgery, cancer, tissue injury, and many autoimmune diseases can increase it. The ESR test changes more slowly and is influenced by anemia, age, kidney disease, pregnancy, and immunoglobulin concentrations.
A complete blood count can show:
- Normocytic anemia from inflammation or kidney failure
- A rapid hemoglobin fall from pulmonary or gastrointestinal bleeding
- High platelets during systemic inflammation
- Leukocytosis from inflammation, infection, or corticosteroids
- Eosinophilia, which shifts attention toward EGPA, allergy, parasitic disease, or a drug reaction rather than typical MPA
Anemia deserves careful interpretation. Gradual anemia may reflect chronic inflammation or impaired erythropoietin production. A sudden fall accompanied by breathlessness and new lung opacities can indicate diffuse alveolar hemorrhage even when the patient does not cough visible blood.
Albumin can fall through inflammation, kidney protein loss, poor intake, or gastrointestinal loss. Liver tests are useful before medication and can identify alternative diagnoses. None of these values replaces direct assessment of the affected organ.
CRP and ESR may normalize with treatment while residual vascular injury remains. They can also rise because of infection during immunosuppression. A relapse diagnosis should therefore require compatible clinical or objective organ evidence, not an isolated laboratory increase.
Lung and Other Organ Assessment
Pulmonary capillaritis is the other major emergency in MPA. It can cause diffuse alveolar hemorrhage, with shortness of breath, falling oxygen, anemia, and bilateral lung opacities. Hemoptysis may be absent. Chest imaging, pulse oximetry or arterial blood gas, serial hemoglobin, and sometimes bronchoscopy are more informative than antibody levels in the acute setting.
MPO-ANCA-associated disease can also coexist with interstitial lung disease. Fibrotic lung changes may precede systemic vasculitis by years, develop alongside it, or progress independently. High-resolution CT and pulmonary function tests help distinguish inflammatory infiltrates, hemorrhage, infection, and chronic fibrosis.
Other organ clues include:
- Palpable purpura, ulcers, or livedo from skin-vessel inflammation
- Foot drop, wrist drop, burning pain, or asymmetric numbness from mononeuritis multiplex
- Muscle and joint pain
- Abdominal pain or gastrointestinal bleeding
- Eye inflammation, though destructive orbital or sinus disease is more characteristic of GPA
- Constitutional symptoms such as fever, night sweats, poor appetite, and weight loss
Nerve-conduction studies and electromyography can document peripheral nerve injury. Skin or nerve biopsy may provide diagnostic tissue when kidney biopsy is not possible. The site selected should balance safety with the likelihood of capturing active disease.
Upper-airway granulomatous features—chronic destructive sinusitis, saddle-nose change, subglottic stenosis, or lung nodules with cavitation—favor GPA over MPA, although overlap exists. Classification labels should not delay treatment of the shared organ-threatening syndrome.
Biopsy and Conditions That Mimic MPA
Kidney biopsy is the diagnostic gold standard when glomerulonephritis is present and the procedure is safe. Light microscopy may show focal segmental necrosis and crescents. Immunofluorescence typically shows few immune deposits, and electron microscopy helps exclude immune-complex disease.
The biopsy also separates active, potentially reversible lesions from chronic scarring. Cellular crescents and necrosis indicate active injury; fibrous crescents, global glomerulosclerosis, interstitial fibrosis, and tubular atrophy indicate established damage. This distinction helps clinicians discuss the likelihood of kidney recovery.
In a patient with a rapidly worsening compatible syndrome and positive MPO- or PR3-ANCA, guidelines advise that waiting for biopsy should not delay immunosuppressive treatment. Biopsy should still be obtained when feasible because it can uncover an unexpected diagnosis, document chronicity, and guide prognosis.
Important mimics include:
- Anti-GBM disease, including double-positive ANCA and anti-GBM disease
- Lupus nephritis and other immune-complex glomerulonephritides
- IgA vasculitis or IgA nephropathy
- Infection-related glomerulonephritis and infective endocarditis
- Cryoglobulinemic vasculitis
- Thrombotic microangiopathy
- Cholesterol embolization
- Drug-induced interstitial nephritis or vasculitis
- Malignancy-associated inflammation
Complement is usually normal in uncomplicated ANCA-associated vasculitis. Low C3 or C4 is not impossible, but it should prompt a careful search for infection, lupus, cryoglobulins, or another immune-complex process. The C3 and C4 panel therefore functions mainly as a differential-diagnosis tool.
Blood cultures and echocardiography are critical when endocarditis is plausible, because infection can generate ANCA and kidney inflammation. Giving powerful immunosuppression before recognizing infection can be dangerous.
Monitoring and Common Result Patterns
After diagnosis, monitoring should track organs, treatment toxicity, and relapse risk. Kidney follow-up includes creatinine, eGFR, urine blood, urine protein, blood pressure, electrolytes, and sometimes repeat microscopy. Lung disease may require oxygen assessment, imaging, hemoglobin, and pulmonary function testing.
MPO-ANCA can decline with treatment or become negative, but the relationship to relapse is imperfect. A rising or returning titer may increase concern in some patients, especially when kidney disease was the original phenotype, yet treatment should not be intensified on the antibody trend alone. Persistent positivity can remain during remission.
Common patterns include:
MPO-ANCA positive, active urine sediment, rising creatinine: Strongly suggests MPO-associated glomerulonephritis after infection and other mimics are assessed. Urgent nephrology and rheumatology care and kidney biopsy are usually appropriate.
MPO-ANCA positive, normal urine and stable organs: Does not establish MPA. Review symptoms, drug exposures, infection, assay strength, and why testing was ordered.
MPO-ANCA negative, pulmonary-kidney syndrome: MPA remains possible, but anti-GBM disease, immune-complex disease, infection, and other causes require immediate evaluation. Tissue becomes especially valuable.
CRP and ESR rising during treatment: Could represent relapse or infection. Look for objective organ changes before escalating immunosuppression.
Creatinine stable but protein or blood persists in urine: May represent healing, chronic damage, or ongoing low-grade activity. Trends, urine microscopy, and sometimes repeat biopsy help distinguish them.
Creatinine rising with quiet urine: Consider dehydration, medication effects, obstruction, vascular disease, or chronic scarring rather than assuming active glomerulonephritis.
Induction treatment for organ-threatening disease commonly combines glucocorticoids with rituximab or cyclophosphamide; avacopan may reduce glucocorticoid exposure in selected patients. Maintenance therapy follows once remission is achieved. Exact treatment depends on severity, kidney function, fertility considerations, infection risk, prior relapse, and local guidance.
The panel succeeds when it leads to the right action quickly: confirm the antibody pattern, measure organ injury, exclude dangerous mimics, obtain tissue when possible, and never let a reassuring inflammatory marker obscure a deteriorating kidney or lung.
