
The OVA1 test is a blood-based multivariate index assay used to help estimate the likelihood that an ovarian or adnexal mass is malignant before planned surgery. It combines five serum proteins—CA-125 II, transthyretin (prealbumin), transferrin, apolipoprotein A1, and beta-2 microglobulin—into a score from 0 to 10. OVA1 is a triage aid, not a screening test for people without an adnexal mass and not a test that can diagnose ovarian cancer on its own. Its main clinical purpose is to help identify patients whose surgery may be better managed by or in consultation with a gynecologic oncologist. The original assay uses different high-risk cutoffs according to menopausal status: 5.0 or higher for premenopausal patients and 4.4 or higher for postmenopausal patients. OVA1 was designed to favor sensitivity, meaning it catches many malignancies but also classifies a substantial number of benign masses as higher risk. Imaging, symptoms, examination, menopausal status, and ultimately surgical pathology remain essential.
- What it measures: OVA1 combines CA-125 II, transthyretin, transferrin, apolipoprotein A1, and beta-2 microglobulin into one 0–10 risk score.
- Who it is for: Adults with an ovarian or adnexal mass for which surgery is already planned and who have not yet been referred to a gynecologic oncologist.
- Original high-risk cutoffs: ≥5.0 for premenopausal patients and ≥4.4 for postmenopausal patients.
- What a high score means: The mass has a higher likelihood of malignancy and may warrant gynecologic oncology involvement; it does not prove cancer.
- What OVA1 is not: It is not a population screening test, not a stand-alone diagnostic test, and not a replacement for imaging or pathology.

What the OVA1 Test Is
OVA1 is an in vitro diagnostic multivariate index assay. Instead of interpreting one tumor marker at a time, the test measures five serum proteins and uses a proprietary algorithm to generate a single numerical result.
The U.S. Food and Drug Administration cleared the original OVA1 test in 2009 for a specific preoperative use. It is intended for women over age 18 who have an ovarian adnexal mass, are already planning surgery, and have not yet been referred to an oncologist. The result supplements the physician’s independent clinical and radiologic assessment.
That intended use is crucial. OVA1 does not answer, “Do I have ovarian cancer?” It helps answer a narrower question: “For a patient with a mass already headed to surgery, is the risk of malignancy high enough that specialist referral should be strongly considered?”
This matters because ovarian cancer outcomes are generally better when appropriate staging and cytoreductive surgery are performed by clinicians with gynecologic oncology expertise. A triage test is designed to reduce the chance that an unexpected malignancy is first encountered in a setting without that expertise.
OVA1 also differs from genetic risk testing. It does not assess BRCA1, BRCA2, Lynch syndrome genes, or inherited cancer susceptibility. It measures current serum proteins associated with the biologic pattern of a pelvic mass.
Who Should Have OVA1
OVA1 is appropriate only within its defined clinical pathway. The patient already has an adnexal mass found through examination or imaging, and a clinician has decided that surgery is planned.
The test is not designed for:
- Routine screening in people without an adnexal mass
- Screening people solely because of family history or genetic risk
- Deciding whether an adnexal mass exists
- Replacing ultrasound, CT, or MRI characterization
- Confirming ovarian cancer without tissue diagnosis
- Monitoring ovarian cancer after treatment
- Deciding by itself whether surgery is necessary
An adnexal mass can arise from the ovary, fallopian tube, or nearby structures. Most adnexal masses are benign, especially in premenopausal patients. Ultrasound features, symptoms, age, menopausal status, and laboratory testing help estimate risk before surgery.
A single marker such as the CA-125 blood test can be helpful but has limitations. CA-125 may be normal in some early ovarian cancers and elevated in benign conditions such as endometriosis, fibroids, menstruation, pregnancy, and inflammation. OVA1 was developed to increase sensitivity by combining CA-125 with four additional proteins.
The intended-use restriction is clinically important. OVA1 was developed for adults with an adnexal mass for which surgery is already planned, as an aid to deciding whether gynecologic oncology involvement should be considered before the operation. It is not meant to decide whether an asymptomatic person needs surgery, and it is not a substitute for the clinician’s preoperative assessment.
That timing matters because outcomes can differ when an ovarian malignancy is initially managed by a team experienced in gynecologic oncology. If cancer is found, appropriate staging, cytoreduction, and surgical planning may need to happen during the first operation. OVA1 therefore aims to reduce the chance that a malignancy considered low risk preoperatively reaches surgery without the appropriate specialist resources available.
The test is also not a hereditary-risk assay. A person with a strong family history or a pathogenic BRCA1 or BRCA2 variant may need a separate high-risk screening and prevention plan even if no adnexal mass is present. Conversely, a high OVA1 score does not imply an inherited cancer syndrome.
OVA1 Biomarkers and Score
OVA1 measures five proteins that change in different directions in malignant and benign conditions. The algorithm combines them rather than asking the clinician to interpret each result separately.

The final result is reported on a 0 to 10 scale. A higher score corresponds to a higher probability that malignancy is present within the test’s intended-use population.
The original classification thresholds are:
- Premenopausal: score of 5.0 or higher is classified as higher risk.
- Postmenopausal: score of 4.4 or higher is classified as higher risk.
These thresholds should not be read as “percent chance of cancer.” A score of 7 does not mean a 70% cancer probability. The score is an algorithmic index, and predictive value depends on the population being tested and the prevalence of malignancy.
One component, beta-2 microglobulin, can be influenced by kidney function and inflammatory conditions when measured on its own. OVA1 accounts for the overall multivariate pattern rather than treating any one component as decisive.
Understanding OVA1 Results
A higher-risk OVA1 result means the biomarker pattern is more concerning for malignancy and should be combined with the clinician’s evaluation to guide referral and surgical planning. It does not establish a cancer diagnosis.
A lower-risk result means the biomarker pattern is less concerning, but it does not guarantee that the mass is benign. No blood test can replace surgical pathology when tissue is removed.

Menopausal status matters because the test uses different cutoffs. It also matters clinically because the baseline probability and differential diagnosis of an adnexal mass change after menopause.
The physician should not ignore highly suspicious imaging because an OVA1 score is low. Likewise, a high score in a mass with otherwise benign features can represent a false positive. The result is an additional risk signal, not an override of the rest of the evaluation.
Accuracy, Strengths, and Limitations
OVA1 was deliberately designed to favor high sensitivity. In published studies, it has detected more ovarian malignancies than CA-125 alone, including cancers in which CA-125 was not elevated. Reviews commonly report sensitivity around the low-to-mid 90% range when used as intended.
The tradeoff is lower specificity. Many benign masses can receive higher-risk scores. Depending on the study and population, specificity has often been around 40% to 55%. A test with high sensitivity and modest specificity reduces false negatives but creates more false positives.
That design makes sense for a referral tool. Missing a malignancy before surgery may deprive a patient of optimal specialist care, while a false-positive referral generally leads to additional specialist assessment rather than an automatic cancer treatment.
Why performance numbers vary
Sensitivity, specificity, positive predictive value, and negative predictive value change with:
- Menopausal status
- Cancer prevalence in the tested population
- Tumor type and stage
- How suspicious the mass was before testing
- Which clinical assessment is combined with the assay
- Study design and referral setting
Predictive values from a surgical population should not be applied to screening healthy people. This is one of the main reasons OVA1 should not be repurposed as a general ovarian cancer screening blood test.
The test also does not identify tumor histology. A higher-risk score cannot distinguish high-grade serous carcinoma from endometrioid, mucinous, borderline, metastatic, or other tumor types. Pathology makes that diagnosis.
A useful way to understand the accuracy tradeoff is to think about the consequence the test is trying to avoid. For preoperative triage, a false negative may be more consequential than a false positive because an unsuspected malignancy could be operated on without gynecologic oncology involvement. Designing for high sensitivity helps reduce that risk. The cost is that more benign masses will be labeled higher risk and referred for specialist assessment.
This does not mean a higher-risk OVA1 result has the same predictive value in every clinic. If malignancy is uncommon in the tested population, many positive results will still be benign. If the population has a higher pretest prevalence because imaging and clinical features are already concerning, the chance that a high score reflects malignancy rises. That is why published positive predictive values should not be copied directly onto an individual result without knowing the study population.
OVA1 is also not intended as a serial response marker. Once surgery and pathology establish the diagnosis, follow-up is based on the specific tumor type, stage, treatment, imaging, symptoms, and validated markers for that disease. Repeating OVA1 after treatment does not provide the same kind of tumor trend information as following CA-125 in selected ovarian cancers.
OVA1 vs. Other Ovarian Tests
OVA1 belongs to a larger group of methods used to assess adnexal masses. These tools answer overlapping but not identical questions.
CA-125 is a single serum marker. It is widely used but has limited specificity and can miss malignancies, especially some early-stage and non-serous tumors.
HE4 and ROMA use a different biomarker strategy. ROMA combines CA-125, HE4, and menopausal status to estimate risk. Performance varies by population, and local practice determines which tool is preferred.
Overa, also called a second-generation multivariate index assay, replaced some OVA1 proteins with HE4 and FSH and was designed to improve specificity while maintaining high sensitivity. It should not be confused with the original five-marker OVA1 score or its original cutoffs.
Ultrasound-based systems such as expert subjective assessment, IOTA models, or structured radiology categories characterize the mass directly. High-quality ultrasound remains central to adnexal mass evaluation and may perform extremely well in experienced hands.
No one method is best in every setting. The choice depends on imaging expertise, menopausal status, local referral pathways, assay availability, and how the result will change management.
The choice among OVA1, ROMA, Overa, or an ultrasound-based approach also depends on what has already been done. Repeating multiple risk algorithms that answer the same preoperative triage question does not necessarily make the final decision more accurate. The most useful test is one with a defined place in the local pathway and a result that will actually change referral or operative planning.
In practice, clinicians should avoid treating discordant tests as a contest in which one number automatically wins. A low OVA1 score with a highly suspicious solid vascular mass, ascites, or metastatic-appearing disease still requires specialist assessment. A high score with reassuring imaging may prompt review of benign explanations and expert imaging rather than an assumption of cancer.
What Happens After the Result
The practical purpose of OVA1 is to improve preoperative triage. If the result is higher risk, the clinician may refer the patient to a gynecologic oncologist or coordinate surgery at a center prepared for comprehensive staging and cytoreduction if malignancy is found.
The next steps can include:
- Reviewing the ultrasound or other imaging in detail.
- Confirming menopausal status and clinical risk factors.
- Discussing OVA1 as one part of the risk assessment rather than a diagnosis.
- Referring to gynecologic oncology when the overall risk supports it.
- Proceeding with planned surgery using the appropriate surgical team and setting.
- Using pathology from the removed tissue as the definitive diagnosis.
A high score should not trigger chemotherapy without a diagnosis. A low score should not cancel surgery that is otherwise indicated or override worrisome imaging.
The result is most useful when it changes who performs the operation and where it is performed. If the overall assessment suggests a meaningful chance of malignancy, involving a gynecologic oncologist before the first operation can help ensure that appropriate staging or cytoreductive procedures are available if cancer is found. Conversely, a lower-risk result may support the existing surgical plan when imaging and clinical findings are also reassuring. In both directions, OVA1 is an adjunct to preoperative judgment rather than a substitute for it.
Patients can ask useful questions such as: What did my ultrasound show? Is my OVA1 score above the cutoff for my menopausal status? How does the score change the choice of surgeon or hospital? Are there other reasons my biomarkers could be abnormal? And will a gynecologic oncologist be involved if cancer is suspected?
Seek prompt medical attention for severe sudden pelvic pain, fainting, heavy bleeding, fever with significant pain, or rapidly worsening abdominal symptoms. Those problems may reflect torsion, rupture, hemorrhage, infection, or another urgent complication of an adnexal mass and should not wait for a tumor-marker interpretation.
