
The high-sensitivity C-reactive protein test, usually written as hs-CRP, measures very small amounts of C-reactive protein in the blood. The liver produces this protein when inflammatory signals are active in the body. Standard CRP testing is mainly used to investigate infections or inflammatory diseases, while hs-CRP testing can detect lower-grade inflammation associated with cardiovascular risk.
An hs-CRP result does not show whether an artery is blocked, diagnose heart disease, or identify the source of inflammation. Its main value is as one part of a broader risk assessment that includes age, blood pressure, cholesterol, diabetes, smoking, family history, and other health information. Temporary issues such as a respiratory infection, dental inflammation, injury, or strenuous exercise can raise the result. For that reason, an unexpectedly high value often needs to be repeated when the person is well.
- Below 1.0 mg/L generally indicates lower relative cardiovascular risk, although it does not rule out atherosclerosis or other heart risks.
- A result from 1.0 to 3.0 mg/L is traditionally classified as average relative cardiovascular risk.
- Above 3.0 mg/L suggests higher relative cardiovascular risk when the elevation is persistent and no acute inflammatory condition is present.
- An hs-CRP of 2.0 mg/L or higher may act as a cardiovascular risk-enhancing factor during decisions about preventive treatment.
- Results above 10 mg/L usually require evaluation for infection, injury, or another active inflammatory process before cardiovascular interpretation.

What the hs-CRP test measures
The hs-CRP test measures C-reactive protein in milligrams per liter of blood, written as mg/L. CRP is an acute-phase protein made mainly by the liver in response to inflammatory messengers such as interleukin-6. Its concentration can rise when the immune system reacts to infection, tissue injury, chronic inflammatory disease, or metabolic stress.
“High sensitivity” describes the measurement method, not a different type of protein. Both standard CRP and hs-CRP tests measure the same substance. The difference is the concentration range each method is designed to measure accurately.
A standard C-reactive protein test is commonly used when clinicians expect substantial inflammation. An hs-CRP assay measures much lower concentrations with enough precision to evaluate low-grade inflammation in people who may otherwise feel well.

The result reflects inflammation anywhere in the body, not inflammation specifically inside a coronary artery. A person may have a high hs-CRP because of gum disease, arthritis, obesity, smoking, an infection, or another condition unrelated to atherosclerosis. Conversely, a low result does not prove that the arteries are free of plaque.
This lack of specificity is why hs-CRP works best as a risk marker rather than a stand-alone diagnostic test. It can add information to a cardiovascular discussion, but clinicians must interpret it alongside the person’s symptoms, medical history, examination, and other test results. The term hs-CRP refers mainly to the assay’s ability to measure low concentrations accurately rather than to a biologically separate form of CRP.
hs-CRP ranges and result interpretation
The traditional cardiovascular categories classify hs-CRP below 1.0 mg/L as lower relative risk, 1.0 to 3.0 mg/L as average relative risk, and above 3.0 mg/L as higher relative risk. These categories describe relative risk among groups of people. They do not provide an individual diagnosis or directly state the chance of having a heart attack.

Another clinically important threshold is 2.0 mg/L. Cardiovascular prevention guidance has used persistent hs-CRP at or above this level as a risk-enhancing factor. This means the result may support more intensive prevention when a person’s treatment decision is otherwise uncertain. It does not mean that everyone with a result of 2.0 mg/L needs medication.
For example, consider two people who both have an hs-CRP of 2.7 mg/L:
- One is 30 years old, does not smoke, exercises regularly, has normal blood pressure, and has favorable cholesterol levels.
- The other is 58 years old, has high blood pressure, smokes, has an elevated LDL cholesterol level, and has a parent who had an early heart attack.
The same hs-CRP result has very different practical meaning in these two situations. The second person has several established risk factors that carry more weight than hs-CRP alone.
A laboratory may flag a result as “high” according to its own reference interval. That flag does not necessarily use the same cutoffs as cardiovascular guidelines. The ordering clinician should confirm whether the test was performed for cardiovascular risk, active inflammation, or another reason.
What a low hs-CRP means
A low hs-CRP is generally favorable. It suggests that this particular marker is not detecting substantial systemic inflammation at the time of the blood draw. It cannot rule out high LDL cholesterol, coronary plaque, high blood pressure, diabetes, inherited risk, or a previous silent cardiovascular event.
Very low values usually do not require treatment. There is no established benefit to trying to lower an already low hs-CRP further.
What a high hs-CRP means
A high result means that inflammatory signaling is present, but the test cannot identify why. A single mildly elevated value may result from a temporary problem. A persistent elevation is more useful for long-term risk assessment and may also prompt evaluation for chronic inflammatory, metabolic, dental, or lifestyle-related contributors.
The number should not be interpreted as a severity score for heart disease. An hs-CRP of 6 mg/L does not mean that someone has twice as much arterial disease as a person with a result of 3 mg/L.
How hs-CRP relates to heart disease risk
Atherosclerosis is more than cholesterol collecting inside an artery. It is an active process involving cholesterol-containing particles, immune cells, inflammatory signals, tissue repair, and changes in the arterial wall. Inflammation can help plaques form, grow, and become unstable.
Because hs-CRP reflects systemic inflammatory activity, persistently elevated levels are associated with a higher rate of heart attack, ischemic stroke, and other cardiovascular events across populations. The association can remain after researchers account for several traditional risk factors. Research also suggests that hs-CRP, LDL cholesterol, and lipoprotein(a) can provide complementary information because they represent different aspects of cardiovascular risk.
However, association does not mean that CRP itself is always the direct cause of arterial damage. CRP may partly act as a signal produced in response to inflammatory pathways that contribute to atherosclerosis. A high value is therefore best viewed as a warning marker rather than proof that CRP molecules are directly damaging the heart.
Residual inflammatory risk
Some people continue to experience cardiovascular events even after LDL cholesterol has been lowered effectively. When persistent vascular inflammation appears to contribute to this remaining risk, clinicians may call it residual inflammatory risk.
This concept is most relevant to people who already have atherosclerotic cardiovascular disease, such as a previous heart attack, stroke caused by atherosclerosis, or coronary artery procedure. In these patients, persistent hs-CRP elevation may identify a higher-risk pattern, but it does not automatically determine a specific treatment.
Inflammatory risk and cholesterol-related risk can exist together. A person may have:
- High LDL cholesterol with low hs-CRP
- Low LDL cholesterol with high hs-CRP
- High levels of both
- Low levels of both but significant risk from smoking, diabetes, hypertension, family history, or established plaque
This is one reason clinicians may consider hs-CRP alongside a lipid panel rather than using either result in isolation. Other markers, including fibrinogen and cardiovascular risk, may sometimes provide additional context but are not substitutes for a complete clinical assessment.
Does lowering hs-CRP prevent heart attacks?
Some treatments associated with fewer cardiovascular events also reduce hs-CRP. Statins are the best-known example because they lower LDL cholesterol and often reduce inflammatory activity. In the JUPITER trial, rosuvastatin reduced cardiovascular events among selected people who did not have known cardiovascular disease, had LDL cholesterol below the trial’s entry limit, and had hs-CRP of at least 2 mg/L.
That trial does not mean that hs-CRP alone should determine whether someone starts a statin. Treatment decisions consider the person’s total cardiovascular risk, potential benefit, possible adverse effects, preferences, and current prevention guidance.
People should not take aspirin, prescription anti-inflammatory medication, or supplements solely to lower an hs-CRP number. Aspirin can cause serious bleeding, and many anti-inflammatory drugs have cardiovascular, kidney, gastrointestinal, or immune-related risks.
Who may benefit from hs-CRP testing
hs-CRP testing is most useful when the result could reasonably change a preventive decision. It may provide additional information when a person’s calculated cardiovascular risk is neither clearly low nor clearly high and the clinician and patient are deciding how aggressively to manage that risk.
Situations in which testing may be considered include:
- A borderline or intermediate estimated risk of atherosclerotic cardiovascular disease
- Uncertainty about starting or intensifying statin treatment
- A strong family history of early cardiovascular disease not fully reflected in a standard risk score
- Metabolic syndrome, abdominal obesity, or other features associated with chronic low-grade inflammation
- Suspected residual inflammatory risk in someone with established cardiovascular disease
- Cardiovascular events or substantial concern despite apparently acceptable cholesterol levels
An hs-CRP test is not required for every healthy adult. Traditional risk factors—especially age, smoking, blood pressure, LDL cholesterol, diabetes, kidney disease, and established cardiovascular disease—usually guide prevention more directly.
When uncertainty remains, a clinician may compare hs-CRP with other risk-enhancing information or consider tests that answer different questions. A coronary artery calcium scan, for example, can show calcified coronary plaque, while hs-CRP cannot show plaque or its location. The choice depends on the individual’s age, risk level, medical history, and whether the result would change management.
When hs-CRP is unlikely to help
The test may be difficult to interpret or unnecessary when:
- The person currently has a fever, infection, injury, or inflammatory flare
- Surgery or significant trauma occurred recently
- Cardiovascular risk is already high enough that preventive treatment is clearly indicated
- Established cardiovascular disease already requires intensive risk reduction
- The person has symptoms that need urgent evaluation rather than long-term risk testing
- The result would not change any clinical decision
hs-CRP should not be used to diagnose a heart attack. Anyone with new chest pressure, severe shortness of breath, fainting, sudden weakness on one side, facial drooping, or difficulty speaking needs urgent medical assessment regardless of an hs-CRP value.
It is also not a replacement for an inflammatory marker panel when a clinician is evaluating unexplained systemic symptoms. Broader testing may include a complete blood count, metabolic tests, standard CRP, or an erythrocyte sedimentation rate test, depending on the clinical question.
Preparation, timing, and repeat testing
Fasting is usually not required for hs-CRP itself. However, fasting instructions may apply when the blood draw includes triglycerides, glucose, or other tests. Follow the preparation instructions provided by the laboratory or ordering clinician.
Drink water normally unless instructed otherwise. Dehydration is not a standard explanation for a major hs-CRP elevation, but adequate hydration can make blood collection easier.
Before testing, tell the clinician about:
- Recent illness, fever, or vaccination
- Dental infection or recent dental work
- Injury, surgery, or burns
- A flare of arthritis, inflammatory bowel disease, or another inflammatory condition
- Pregnancy or recent childbirth
- Prescription medicines, hormones, and supplements
- Major changes in exercise, weight, smoking, or alcohol intake
Strenuous exercise can temporarily increase inflammatory markers, especially after an unusually intense or prolonged workout. A person seeking a stable baseline should avoid testing immediately after a marathon, demanding competition, or exercise session that caused significant muscle soreness.
Why repeat testing may be necessary
CRP can vary within the same person. One result is a snapshot, and a temporary inflammatory trigger may distort cardiovascular interpretation. When the value is unexpectedly elevated, clinicians often repeat the test after the temporary condition has resolved.
For cardiovascular risk classification, two measurements separated by approximately two weeks have historically been used when the initial result could affect management. The person should be clinically stable and free of obvious acute infection during both tests.
A repeat test is especially important when the result is above 10 mg/L. Rather than assigning the person to a cardiovascular risk category based on that number, the clinician usually looks for an active cause and repeats the test later.
Season, time of day, body weight, smoking, hormonal factors, and minor illnesses may contribute to variation. This does not make the test useless, but it means that trends and clinical context matter more than small decimal changes. A shift from 2.4 to 2.7 mg/L may simply reflect normal biological and laboratory variation rather than meaningful worsening.
Causes of a high or changing hs-CRP
An elevated hs-CRP is nonspecific. Cardiovascular inflammation is one possibility, but many other conditions can raise the result.
Temporary causes
Short-term increases may occur with:
- Respiratory, urinary, skin, or dental infections
- Fever or a recent viral illness
- Injury, burns, or surgery
- Strenuous or unfamiliar exercise
- An inflammatory disease flare
- Recent tissue damage
- Pregnancy, particularly later in pregnancy
- The early recovery period after childbirth
These factors are especially important when a person who usually has a low result suddenly receives a much higher value.
Persistent causes
Longer-lasting elevations may be associated with:
- Smoking or regular exposure to tobacco smoke
- Obesity, especially excess abdominal or visceral fat
- Poorly controlled diabetes or insulin resistance
- Chronic periodontal disease
- Rheumatoid arthritis and other inflammatory disorders
- Chronic kidney disease
- Inflammatory bowel disease
- Some chronic infections
- Obstructive sleep apnea
- Certain cancers
- Ongoing cardiovascular and metabolic inflammation
A high result does not prove that any condition on this list is present. The person’s symptoms and medical history determine which possibilities deserve investigation.
Other laboratory findings can help distinguish broad patterns. A clinician investigating systemic inflammation may review a ferritin blood test, blood cell counts, kidney and liver tests, or disease-specific markers. Ordering many tests without a clinical reason can produce incidental abnormalities and confusion, so evaluation should remain targeted.
Medicines and hormonal factors
Statins often lower hs-CRP in addition to lowering LDL cholesterol. Weight loss treatments, improved diabetes control, and effective treatment of an inflammatory disease may also reduce the result indirectly.
Oral estrogen therapy can raise CRP measurements, including in some people using combined oral contraceptives or menopausal hormone therapy. This effect does not necessarily mean that the person has an infection or a matching increase in arterial inflammation. The medication route and the complete cardiovascular profile matter.
Corticosteroids and other immune-modifying medicines can alter inflammatory markers. Never stop a prescribed medicine merely to obtain a different hs-CRP result unless the prescribing clinician gives specific instructions.
What can cause a low result?
A low hs-CRP commonly reflects low inflammatory activity and is usually reassuring. Statin use, regular physical activity, smoking cessation, improved sleep, weight loss when medically appropriate, and control of chronic disease may contribute.
Some diseases do not consistently produce a strong CRP response. A low value therefore cannot exclude every autoimmune, inflammatory, infectious, or cardiovascular condition. Symptoms and disease-specific tests remain more important when a particular illness is suspected.
What to do after an abnormal result
The first step is to determine whether the result represents a stable baseline. Review recent infections, dental symptoms, injuries, inflammatory flares, strenuous exercise, medicines, and other factors that could have influenced the test. If a temporary cause is likely, the clinician may repeat hs-CRP after recovery.
For a persistent result between approximately 2 and 10 mg/L, the next step is usually a broader cardiovascular assessment rather than a search for a medicine that treats the number directly.
Useful information may include:
- Blood pressure and whether it is controlled
- LDL, HDL, non-HDL cholesterol, and triglycerides
- Diabetes status, fasting glucose, or hemoglobin A1c
- Smoking and nicotine exposure
- Kidney function
- Family history of early heart attack or stroke
- Physical activity, sleep, weight pattern, and diet
- Known coronary, carotid, or peripheral artery disease
- Other risk-enhancing conditions or biomarkers
Practical ways to reduce overall risk
The most effective strategy is to address proven cardiovascular risks while investigating treatable causes of inflammation.
- Stop smoking and avoid secondhand smoke. Smoking promotes vascular injury and inflammation and substantially raises cardiovascular risk.
- Exercise consistently. A practical goal for many adults is at least 150 minutes of moderate aerobic activity each week, plus muscle-strengthening activity on two or more days. Medical guidance may be needed before starting if symptoms or significant disease are present.
- Improve dietary quality. Emphasize vegetables, fruit, legumes, whole grains, nuts, seeds, fish, and unsaturated fats. Limit processed meat, refined carbohydrates, trans fats, and heavily processed foods.
- Manage blood pressure, cholesterol, and diabetes. These established risk factors usually provide clearer treatment targets than hs-CRP itself.
- Treat chronic inflammatory and dental conditions. Effective treatment may reduce both symptoms and systemic inflammatory activity.
- Address sleep and possible sleep apnea. Loud snoring, witnessed pauses in breathing, morning headaches, and excessive daytime sleepiness deserve medical review.
- Aim for sustainable weight management when appropriate. Loss of excess visceral fat can improve metabolic health and may lower hs-CRP, but the plan should account for nutrition, medications, mobility, and other health conditions.
Medication decisions should be personalized. A persistent hs-CRP of at least 2 mg/L may strengthen the case for a statin in someone whose overall risk assessment leaves the decision uncertain. It should not override contraindications, side-effect concerns, or the person’s preferences.
An hs-CRP result should also not lead to self-treatment with aspirin. Preventive aspirin decisions require careful consideration of age, cardiovascular risk, bleeding risk, ulcer history, kidney function, and concurrent medicines.
The most useful question is not simply, “How do I lower this number?” It is, “What is causing the elevation, what is my complete cardiovascular risk, and which proven steps are most likely to improve my health?” Used in that context, hs-CRP can help refine a prevention plan without being mistaken for a diagnosis.
